摘要
用ELISA方法检测结核性和非结核性(癌性及漏出液)胸水及血清中的可溶性白细胞介素2受体(sIL-2R)。结果显示:结核组sIL-2R水平明显增高(2464±1001U/ml),与癌性胸腹水组(752±418U/ml)及漏出液组(410±315U/ml)比较有显著性差异(P<0.001)。与胸水中腺苷脱氨酶活性(ADA)呈正相关(r=0.688,P<0.001)。敏感性及特异性均为90%以上。提示用本法检测胸腹水的sIL-2R对于结核性胸腹水的诊断及鉴别诊断有重要意义。
he levels of soluble IL-2R were determined in pleural
fluid and serum from 30 patientswith tuberculous pleurisy and 57
nontuberculous(30 cases of malignant,27 cases of transudative,There
were significantly elevated soluble IL-2R values in tuberculous
pleural fluid(2464±1001U/ml)as compared with pleuroperitoneal fluid
of nontuberculous etiology including malig-nant(752± 418U/ml)and
transudative(4l0± 315U/ml)effusions.In patients with tubercu-lous
pleurisy,the level of soluble IL-2R in pleural fluid was markedly
greater than that inserum.A significant positive correlation was
observed between soluble IL-2R levels and adeno-sine deaminase levels
in tuberculous pleural fluid.Fixing the cut off points of soluble
IL-2R val-ues at 1500U or l000U the tuberculous pleural effusion
cases were differentiated from the non-tuberculous with a
sensitivity、specificity、postive predictive value、negative
predictive value andaccuracy of over 90 percent.
