2型糖尿病肾病的相关危险因素 被引量:6

摘要 目的 分析2型糖尿病肾病进展的相关危险因素。 方法 12 1例确诊2型糖尿病患者根据尿微量白蛋白水平分3组:单纯糖尿病组、早期糖尿病肾病组及临床糖尿病肾病组,测定性别、年龄、病程、血β2 微球蛋白、血压、血脂、空腹及餐后2h血糖、胰岛素和C肽值、HbA1c、肾小球滤过率、活化血小板(CD62p/CD63 )、纤维蛋白原。采用HOMA公式计算胰岛素抵抗指数。 结果 通过逐步判别分析,从2 3个变量中筛选出对糖尿病肾病进展最有影响的强相关危险因素:CD62 p/CD63 、血β2 微球蛋白、病程、纤维蛋白原、舒张压、糖化血红蛋白、空腹血糖、甘油三酯。 结论 血糖、血压控制不良,脂代谢紊乱,病程延长,高凝状态,血小板高度激活皆为糖尿病肾病发生与进展的独立危险因素。
