摘要
目的探索及评估一种更理想的置换液配方。方法将两种自配方和使用方法与Port配方进行比较,分动物实验和临床观察两部分,比较其在连续血液净化(CBP)中对电解质、渗透压及酸碱平衡维持的优劣。结果两种自配方的葡萄糖浓度、总分子浓度分别为13.8mmol L、307.3mmol L,比Port配方的59.1mmol L、351mmol L更接近生理状况;动物实验显示自配方组3h血糖、血钙浓度分别为(7.2±0.7)mmol L、(7.4±0.8)mmol L及(2.3±0.2)mmol L、(2.3±0.5)mmol L,明显好于Port方组的(14.2±1.0)mmol L、(1.8±0.2)mmol L(P<0.01);治疗中Port方组渗透压显著上升[由(298.0±8.0)mOsm L至(317.0±8.0)mOsm L,P<0.05],自配方组无显著变化;临床资料也显示了相似的变化趋势;自配方2组使用更方便。结论两种自配方在血糖、血钙及渗透压调控方面较优于Port方,其中自配方2在易操作性方面又优于自配方1,似有进一步验证和应用的价值。
Objective To compare two continuous blood purification. Methods We compared the new substitutions used in our hospital with the Port's one by testing the electrolytes, glucose concentration and osmolarity in animals' or patients' blood before and during CBP. Results The glucose, calcium plasma concentration and the osmolarity of animal [(7.2±0.7)mmol/L, (2.3±0.2)mmol/L,(306.0±10.0)mOsm/L] and patients (11.1±3.8)mmol/L, (2.0±0.3)mmol/L,(325.0±7.0)mOsm/L,respectively]after using the new substitutions were quite different from those after using the new substitutions were quite different from those [animal,(14.2±1.0)mmol/L,(1.8±0.2)mmol/L,(317.0±8.0)mOsm/L, patients(16.7±3.8)mmol/L, (1.6±0.2)mmol/L,(333.0±8.0)mOsm/L,respectively]after using the old one at the same time point during CBP (P<0.05). Conclusion The new substitution compositions may be better than the port's one in dominating the plasma concentration of glucose and calcium and plasma osmolarity.
