摘要
[目的]观察参芪合剂联合微波凝固治疗原发性肝癌(HCC)近期疗效。[方法]将72例HCC患者随机分为两组,治疗组(参芪合剂加微波治疗)36例;对照组(单纯微波治疗)36例。运用流式细胞仪检测外周血T淋巴细胞亚群、自然杀伤(NK)细胞的表达水平;用放射免疫法检测血清甲胎蛋白(AFP)定量;检查肝脏B超、CT及肝肾功能;观察患者治疗前后临床主症的变化及不良反应。[结果]治疗组和对照组的显效、有效、无效、恶化分别为3、24、4、5和1、19、9、7例,治疗组总有效率(75.00%)显著优于对照组(55.56%,P<0.05);两组治疗后AFP比治疗前均有显著下降(P<0.01),且治疗组下降程度及各项免疫指标(CD3+、CD4+、CD4+/CD8+、NK)改善程度均优于对照组(均P<0.01)。[结论]参芪合剂联合微波凝固可以有效治疗肿瘤,增强免疫功能,是对失去手术治疗机会HCC的有效治疗方法之一。
Objective] To evaluate the short term clinical effect in treating hepatocellular carcinoma combined percutaneous microwave coagulation therapy (PMCT) with Shenqi-mixture.[Methods] Seventy-two patients suffered with hepatocellular carcinoma (HCC) were randomly distributed into two groups. In the treatment group,36 patients received Shenqi-mixture after PMCT, and 36 patients in the controll group were treated with PMCT simply. T-lymphocyte subsets in the peripheral blood and the activities of natural killer cells were detected with flowcytometry. The serum α-fetal protein(AFP) was detected by radio-immunological technology. At the same time, the observed indexes including the changes of symptoms and signs, side effects, Ultra-sound, CT and liver or kidney functions were considered.[Results] In the treatment group, three cases were evaluated as CR, 24 PR , 3 NC, and 5 PD. Compared with the controll group (CR,PR,NC and PD were 1,19,9 and 7 cases respectively), the response rate was significance with P<0.01. AFP in the treatment group decreased more significant than that in the controll group(P<0.01).The peripheral blood levels of CD3^+, CD4^+, CD4^+/ CD8^+ and NK were higher in the treatment group than that in the controll group(P<0.01).[Conclusion]The treatment for HCC combined Shenqi-mixture with PMCT could not only inactivate the tumor cells effectively, but also improve the immune system function of the patients. It is an effective method for HCC patients who have lost the surgical chance.
