新生儿缺氧缺血性脑病的脑血流改变 被引量:74

MEASUREMENTS OF CEREBRAL BLOOD FLOW IN ASPHYXIATED NEWBORNS BY COLOR DOPPLER IMAGING (CDI)

摘要 应用彩色多普勒检测窒息及正常新生儿脑血流各10例。结果窒息组第1天大脑中,前和后动脉部分血管仅有收缩峰,而无舒张期的单峰型频谱特征,血流参数均降低,与对照组相比,差异有非常显著意义。提示脑血流量减少是窒息后脑损伤的主要原因。9例存活者第2天颅内血管频谱首先恢复正常形态,但血流参数第7天才完全恢复正常。中度缺氧缺血性脑病患儿部分血管阻力指数升高至1,重度患儿持续为1,提示血管阻力指数可作为估价预后的重要指标之—。 Cerebral blood flow (CBF) was serially measured in 10 asphyxiated term infants with evidence of hypoxic-ischemic encephalopathy (HIE) and com-pared with that of normal term infants by CDI. The flow velocity patterns in the anterior, middle and posterior cerebral arteries were studied and the resultsshowed absence of continuous blood flow during diastole in some vessels on day 1, but returned to normal on day 2 in moderate HIE cases. There was a significant decrease in CBF on day 1 compared with control and increased to normal by day 7 in 9 infants. The resistance index (RI) rose to 1 in some vessels of moderate HIE Infants and persisted at 1 in the severe HIE infants. We conclude that low CBF plays a key role in brain damage of asphyxiated newborns and RI may be an important parameter in evaluation of prognosis.
