摘要
目的观察犬心跳骤停复苏后心肌细胞凋亡的变化.方法体外电击诱发犬室颤,3min后复苏,12只犬随机分为2组,CPR组,空白对照组,每组6只,采用Swan-Ganz漂浮导管监测复苏前和后6h的CO和PAWP,6h后取心肌组织,TUNEL法检测心肌组织凋亡的形态学改变.结果两组各血流动力学指标在心跳骤停前无统计学差异,CPR组的MAP在复苏成功即时高于空白对照组,随后开始下降,在复苏后4、6h低于空白对照组.CPR组的PAWP从心跳骤停前的(5.00±1.26)mmHg一直上升,到复苏后达高峰(28.83±4.79)mmHg,各观察点均高于空白对照组,CO在复苏成功后随时间延长而下降,6h降至最低,复苏后各观察点均低于空白对照组.CPR组心肌细胞凋亡指数高于空白对照组.结论电击诱发室颤犬复苏成功后存在着心功能不全和心肌细胞凋亡增加.
ObjectiveTo investigate the changes of myocardial apoptosis in postresuscitation myocardial dysfunction dog from cardiac arrest. MethodsTwelve dogs were randomly divided into Blank group and CPR group (n=6). Ventricular fibrillation (VF) was induced by the external electricity, except for in Normal group.Cardiopulmonary resuscitation was taken after 3 minutes VF. Cardiac output (CO) and pulmonary artery wedge pressure (PAWP) were measured and blood samples were collected in 12 dogs before and during a 6-hour interval after successful resuscitation. Heart tissues were taken out to detect the myocardial apoptosis by terminal deoxynucleotidyl transferase-mediated dUTP nick endlabeling(TUNEL) after a 6-hour experiment. ResultsThe hemodynamic parameters were not significantly different before VF in two groups. The MAP of CPR group was higher than that of normal group at the time of return of spontaneous circulation (ROSC), and then fell. The MAP was lower than those of normal group in 4 h and 6 hour. These parameters of CPR group were lower than those in normal group after successful resuscitation. PAWP of CPR group increased from a prearrest level of 5.00±1.26 mmHg to a maximum of 28.83±4.79mmHg at 6 hours after successful resuscitation, and was higher than that of normal group. CO in descending order of time was lower than that in prearrest and normal group after ROSC. These parameters were decreased to the minimum in 6-hour.The ratio of apoptosis in CPR group was significantly higher than that in Normal group. ConclusionsThere are myocardial apoptosis increasing in postresuscitation dogs from cardiac arrest with Postresuscitation myocardial dysfunction.
