人乳头瘤病毒与液基细胞检测法在宫颈癌前病变诊断中的意义 被引量:7

摘要 并用人乳头瘤病毒杂交捕获法(hc2HPVDNA)和液基细胞检测法(TCT)联合监测宫颈癌前病变,提高诊断率。用hc2HPVDNA和TCT两种方法检测1231例宫颈外口及颈管脱落细胞和分泌物。1231例样本中,HPV感染率34.0%,而21~40岁年龄段略高于其它年龄段。TCT检测:正常+炎症1077例;ASCus32例;ASCh34例;LSIL73例,HSIL15例。ASCus、ASCh、LSIL和HSIL检出率分别为2.6%、2.8%、5.9%和1.2%。在细胞学诊断为ASCus、ASCh、LSIL和HSIL组中,HPV感染率分别为50.0%、58.8%、76.7%和93.3%。统计学检验表明:细胞形态和结构的异常与HPV感染具有高度相关性(p<0.01)。TCT检查证实细胞形态正常者中,HPV感染率占29.0%;当细胞形态和结构出现异常时,HPV的感染率明显增加(HSIL>LSIL>ASCh>ASCus>正常+炎症)。 To raise the diagnosis rate of precancerous cervical diseases with thin-prep cytology test(TCT) and human papilloma virus DNA hybrid capture II(hc2 HPV DNA). 1231 specimens of desquamated cells and secretion from cervical os and canal were treated with TCT and hc2 HPV DNA methods and examined. Among the 1231 speciment of desquamated cervical cells, rate of HPV positive cases was 34.0% and had a slightly high tendency at 21~40 age. With TCT exam, (1 077) specimens showed normal cells or cervicitis, while ASC-us was in 32(2.6%), ASC-h in (34(2.8 %)), LSIL in 73(5.9%) and HSIL in 15(1.2%). In cytologic diagnoses, the positive rates of HPV were: ASC-us 50.0%, ASC-h 58.8%, LSIL 76.7% and HSIL 93.3% respectively. The results revealed a close correspondence(p<0.01) between HPV infection and the abnormal cell morphology and cell construction. With normal cell morphology in TCT exam, the positive cases with HPV infection were 29.0%, while there was an obvious increase of HPV infection when abnormal cellular morphology and structure were present(HSIL > LSI L > ASC- h > ASC-us >normal morphology + cervicitis).
