丘脑底核深部电刺激对帕金森病患者抑郁及焦虑的影响 被引量:6

Effect of deep brain stimulation of subthalamic nucleus on anxiety and depression of patients with Parkinson's disease

摘要 目的探讨丘脑底核(STN)深部电刺激(DBS)手术对帕金森病(PD)患者抑郁及焦虑的影响。方法41例接受STNDBS治疗的PD患者进行神经心理评估,分别于术前1周及术后12个月应用HY分期、统一PD评定量表运动检查部分(UPDRSⅢ)、医院焦虑和抑郁量表(HADS)及PD生活质量问卷(PDQ39)评价其病情、运动状况、情绪及生活质量,并进行统计学分析。结果术后患者UPDRSⅢ、HADS及PDQ39评分较术前极显著改善(均P<0.001),HY分期在“开”及“关”状态均明显改善(P<0.05,P<0.001);Spearman相关性检验提示HADS抑郁评分与PDQ39中情绪状态、羞耻感及总评分变化正相关(r分别为0.35、0.37、0.34,均P<0.05),与HY分期、UPDRSⅢ不相关。HADS焦虑评分与其均不相关。结论STNDBS能改善PD患者的情绪,特别是抑郁状态,进而改善其生活质量。 Objective To evaluate the effect of bilateral subthalamic nuclei deep brain stimulation (STN-DBS) on anxiety and depression of patients with Parkinson's disease(PD).Methods Forty-one consecutive patients with refractory motor fluctuations and dyskinesia were assessed with Hoehn & Yahr scale, Unified Parkinson’s Disease Rating Scale Ⅲ (UPDRSⅢ), HADS, PD Questionnaire Chinese version (PDQ-39) a week before surgery and 12 months after the surgical procedure. Results The scores of UPDRSⅢ, HADS and PDQ-39 significantly increased after STN-DBS treatment (all P<0.001),and Hoehn & Yahr scale also significantly increased(P<0.05,P< 0.001). The score of HADS-Depression was correlated with emotional state, stigma and total score of PDQ-39 by test of Spearman (r was 0.35, 0.37 and 0.34 respectively, all P<0.05), and it was not related with Hoehn & Yahr scale and UPDRSⅢ. No relationship was found between HADS-Anxiety and the other scales.Conclusion STN-DBS significantly improves the emotion (especially in depression) of the patients with advanced PD, hence ameliorates the quality of life of these patients.
