肝局灶性病变CT动脉期强化方式的诊断价值 被引量:1

The evaluation of the enhancement patterns at arterial phase CT in diagnosis of the focal liver lesion

摘要 目的分析肝局灶性病变在CT增强扫描动脉期的强化方式并评价其诊断与鉴别诊断价值。方法回顾性分析200例肝局灶性病变的CT扫描动脉期图像,将其强化方式划分为六类并结合相应诊断结果进行分析。结果96.5%的病灶有动脉期强化,阳性预测值≥83%、特异性≥94%的强化方式有:肝细胞癌的不均匀强化或病灶内异常血管,血管瘤的周边结节状强化,转移瘤的完整环形强化及脓肿的靶状强化。结论上述五种肝局灶性病变的动脉期强化方式在诊断与鉴别诊断上有重要价值。 Objective To analyze the enhancement appearances at arterial phase CT, and evalua the enhancement patterns in diagnosis of the focal liver lesions. Methods Arterial phase CT images were analyzed retrospectively in 200 cases with focal liver lesions were confirmed and the enhancement pattern of the lesions were classified into six categories. Results 96.6% of the 200 lesions demonstrated arterial phase enhancement. Patterns associated with positive predictive values of 83% or greater and specifictiy of 94% or greater included heterogeneous or abnormal internal vesels (hepatoceliular carcinoma), peripheral nodules (hemangioma), sigle ring (metastasis) and multiple ring (abscess). Conclusion These enhancement appearances at arterial phase CT is of great applicative value in diagnosis and differential diagnosis of the focal liver lesions.
