摘要 目的:分析影响TIMI帧数的临床及解剖相关因素。方法:分析280例患者的749支冠状动脉的校正TIMI帧数(CTFC).以及其与性别、年龄、BMI、吸烟史、高血压、糖尿病、冠状动脉血管直径、血管狭窄程度、动脉收缩压、舒张压、心率等因素的相关关系。结果:(1)狭窄<50%的冠状动脉的帧数与冠脉血管直径正相关,与收缩压、心率负相关。相关系数(r)和P值分别为:前降支CTFC与血管直径r=0.312,P=0.002;与收缩压r=-0.241,P=0.019;与心率r=-0.165.P=0.05。回旋支CTFC与血管直径r=0.298,P=0.0001;与收缩压r=-0.267,P=0.001;与心率r=-0.172,P=0.033。右冠与血管直径r=0.210,P=0.05;与收缩压r=-0.274,P=0.001;(2)男性患者的前降支、回旋支、右冠的TIMI帧数分别为23.55±8.90帧;29.77±15.55帧;21.08±9.28帧;女性患者分别为LAD19.97±6.19帧;23.42±9.48帧;17.04±5.99帧,P值分别为0.023、0.001、0.001;3)按冠状动脉狭窄<70%、70%-90%、>90%分组,前降支、回旋支、右冠的三组之间CTFC具有显著差异,P值分别为0.0001、0.001、0.0001。结论:(1)TIMI帧数计数是反应冠状动脉血流较好的连续变量;(2)冠状动脉血管直径与CTFC明显相关;(3)CTFC与冠状动脉的狭窄程度明显相关;(4)男性患者的CTFC明显长于女性患者。 Objective: To evaluate clinical and anatomic factors about corrected thrombolysis in myocardial infarction frame count (CTFC). Methods: The CTFC of 749 coronary arteries have been analysed in 280 patients. The correlation of CTFC with potential affecting factors including sex, age, BMI,smoke,hypertension, diabetes,diameter of coronary artery, severity of stenosis, systolic blood pressure, diastohc blood pressure, heart rate were studied. Results: 1 The CTFC was positively correlated to diameter of coronary artery, and negatively correlated to systolic blood pressure and heart rate in the vessels of stenosis severity less than 50 per cent. The CTFC of left anterior descending (LAD) was correlated to diameter of LAD (r = 0.312, P = 0.002), systolic blood pressure (r = -0.241, P = 0.019), heart rate(r = -0.165, P = 0.05), and CTFC of left circumflex artery (LCX) was correlated to diameter of LCX (r = 0. 298, P = 0.000), systolic blood pressure (r= -0.267, P = 0.001), heart rate (r= - 0. 172, P = 0.033), and CTFC of right coronary artery (RCA) was correlated to dt ameter of RCA (r = 0.210. P = 0. 05), systolic blood pressure (r= -0.274. P = 0. 001). 2 . The CTFC in male patients were 23. 55 ± 8. 90 frames in LAD. 29. 77 ± 15. 55 frames in LCX and 21. 08 ± 9. 28 frames in RCA,but in female patients the CTFC were 19. 97 ± 6. 19 frames in LAD, 23. 42 ± 9. 48 frames in LCX and 17. 04 ± 5. 9 frames in RCA, P values were 0.023, 0.001. 0.001, respectively. 3. Three groups were distrbuted acceding to stenosis seventy (<70%,70%-90% ,>90%), and the CTFC in 3 groups were different significantly. P values were 0.0001 in LAD, 0.001 in LCX.0. 0001 in RCA, respectively. Conclusion:1. The CTFC is a good continuous variable used to assesst coronary blood flow. 2. The CTFC is obviously correlated to diameter of coronary artery . 3. The CTFC is positively correlated to stenosis severity . 4. The CTFC of male patients is obviously longer than that of female patients.
