机械通气在亚低温治疗重型颅脑损伤患者中的应用 被引量:2

摘要 目的:探讨机械通气对亚低温治疗重型颅脑损伤的影响。方法:对急性重型颅脑损伤患者在亚低温治疗中采用机械通气(机械通气组),对比机械通气前后患者各项血气分析指标﹑达到亚低温治疗温度所需时间及颅内压的变化,并与32例正常呼吸组比较。完全机械通气组100例患者以肌松剂阻断了自主呼吸,以呼吸机行完全机械通气;不完全机械通气组68例保留自主呼吸,以同步呼吸模式辅助呼吸。结果:在应用呼吸机后,患者呼吸功能明显改善,血气分析指标基本恢复正常,达到亚低温所需治疗温度的时间显著短于对照组。2组机械通气治疗的患者颅内压明显降低。结论:机械通气改善了重型颅脑损伤患者的通气功能障碍,迅速降低颅内压,减少了继发脑损害。 Objective To study the effect of mechanical ventilation on the patients with severe head injury in mild hypothermia treatment. Methods Severe head injury patients were given mechanical ventilation during mild hypothermia therapy. Changes of blood gas values the time spent for cooling and the influence to intracalvarium pressure were analyzed and compared with those in the normal breathe group. One hundred patients received skelaxin to block automatic breathe for mechanical ventilation. Sixty eight cases remained automatic breathe during mechanical ventilation. Results Mechanical ventilation significantly improved the blood gas values shorten the time spent for cooling and decreased the intracalvarium pressure in patients. Conclusion Mechanical ventilation can improve ventilation disorders decrease the intracalvarium pressure rapidly and decrease the secondary injury in patients with severe head injury.
