摘要
为了探讨视网膜静脉阻塞患者血液流变学改变与气血辨证的内在关系,对49例患者和25例正常对照者进行血液流变学方面的研究。结果显示血液粘度增高和红细胞聚集性增高,其结果与瘀血的形成机理相吻合。在45岁以上患者中,血液流变学改变尤为明显。中央静脉阻塞与分支静脉阻塞比较,完全阻塞与不完全阻塞比较,气虚血瘀型与气滞血瘀型比较,血液流变学各项指标均无明显差异。这一结果说明:不论原发诱因如何,其客观结果皆为瘀血已经形成,成为致盲的因素。注重急症的瘀血证候,不强调原发机制,这和中医“审证求因”的认识是吻合的。
In order to find the inherent relationship between the changes in rheology and the differentiation of syndromes in qi and blood in patients with retinal vein obstruction, we studied 49 cases of the patients and the same number of healthy people as a control group. The results showed that the group of patients with retinal vein obstruction had an increased blood viscosity and an increased erythrocyte aggregation. These results agree with the mechanism of the formation of blood stasis. In patients aged 45 years or older, the changes in rheology were even more significnt. No significant difference in the changes of rheology was found between the central retinal vein obstruction and branch retinal vein obstruction, complete obstruction and incomplete obstruction; stagnancy of blood due to deficiency of qi to stagnancy of both blood and qi. The results indicate that regardless of the causes of the obstruction, the final result is the formation of blood stasis which leads to blindness. So, it is blieved that the symptoms of blood stasis shoud be emphasized, while the orginal mechanism of the disease should be deemphasized. This is identical to the traditional Chinese medical theory of 'determination of pathogenic factors based on the differentiation of symptoms and signs''.
