多支供血的脑动静脉畸形在血管造影上的构筑特征 被引量:1

摘要 根据全脑和超选择脑血管造影,分析了93例多支供血的脑动静脉畸形的血管构筑特征。整个畸形血管团由多个间隔组合而成,每根供血动脉支配一个间隔,每个间隔有各自的一条或数条引流静脉,亦可与其他间隔共用一条引流静脉。此外,注意到动静脉畸形周围的血管瘤样改变。并对其形成机理和临床意义进行了探讨。 Abstract The angioarchitectural features of multisupplied AVMs were analyzed in 93 cases based on pan-cerebral angiography and superselective angiography. The results revealed that AVMs with multiple feeders had multiple; compartments. Each compartment consisted of a single feeder and one or more draining veins of its own, or shared a common draining vein with other compartment. In addition, angiomatous changes beside AVMs were noticed in some cases. The authors suggest that (1) the multicompartmental feature be due to embryonal developmental disturbance of capillary network in the multisulloied border zone of the brain (2) the whole lesion can not be occluded by embolization via a single feeder, and the residual nidus can hardly be spontaneously thrombosed; and (3) angiomatous change beside the nidus of AVM are acquired (or secondary) feeders rather than true vascular malformations.
