摘要
10个钛合金关节(8个髋关节,2个膝关节)假体在植入体内1~13年后发生断裂。检测表明断裂具有典型的应力腐蚀疲劳特征。断裂的主要原因为:原材料成分超标(1例次),存在夹杂、裂隙、疏松等内部缺陷(2例次);设计违背生物力学原理(5例次);假体坯件锻造过热(9例次);表面加工粗糙(2例次);髋关节假体柄置于内翻位(2例次)。尽快制订与国际标准接轨的质量控制标准,对外科植入物实施严格的行政管理,开展手术操作培训是降低断裂率的根本策略。
AbstractTen Ti-6Al-4V joint prostheses,including 8 hipsand 2knees,were fracturedafter 1 to 13 vears in vivoservice following the primary arthroplasty. The metal-lographs and electronic microscopies,taken from theretrieved components, demonstrated that there are typ-ical characteristics of stress corrosion fatique. The ex-cessive chemical content of raw material,the presenceof interior defects, such as inclosures and porosities,in-correct design features,which violate biomechanicprinciples,the overheating and improper surface finish-ing of the parts,and the clinical varus positioning of thehip stem contribute to the device failure. Policies areproposed to overcome this problem. Among them aredrawing national mandatory standards on surgical im-plants,aking strigent administration measures to en-sure the quality of joint prostheses and the establish-ment of training centers for arthroplasty.
