摘要
为了阐明高选迷切加胃窦粘膜切除术（Ｈｉｇｈｌｙｓｅｌｅｃｔｉｖｅｖａｇｏｔｏｍｙ－ｍｕｃｏｓａｌａｎｔｒｅｃｔｏｍｙ，ＨＳＶ－ＭＡ）对胃窦功能和结构的影响，１８只狗被随机分成对照组、ＨＳＶ、ＨＳＶ－ＭＡ组以观测胃窦组织形态学，胃排空、胃窦肌电活动和胃窦压力变化，在ＨＳＶ和ＨＳＶ－ＭＡ组，试验在术后４个月进行。术后在重建的胃窦粘膜内未发现Ｇ细胞再生，通过胃肠钡餐造影检查发现所有三组中犬胃及十二指肠形态正常和有清楚的胃蠕动功能。在ＨＳＶ－ＭＡ组，胃初排及排空时间分别为５．０±０．０６ｍｉｎ，４．０±０．４ｈ，这和对照组及ＨＳＶ组相似（Ｐ＞０．０５），ＨＳＶ－ＭＡ组的胃窦肌电频率为３．１１±０．６５次／ｍｉｎ，明显低于对照组和ＨＳＶ组。肌肉注射五肽胃泌素后所有三组的胃窦肌电频率均明显增加（Ｐ＜０．０１），它们与注射前的差别消失（Ｐ＜０．０５）。胃窦压力波紧接着每个胃窦肌电动作电位后出现。ＨＳＶ－ＭＡ组和对照组的胃窦压力值接近（Ｐ＞０．０５），均明显低于ＨＳＶ组（Ｐ＜０．０５）。在所有三组，肌肉注射五肽胃泌素能明显增加胃窦压力值（Ｐ＜０．００１），并消除三组间在胃窦压力值上的差异。这些结果提示，在ＨＳＶ－ＭＡ组尽管?
In order to observe the effects of high-level selective vagotomy and antral mucosal resection(HSV-AMR)on the functional and histological changes of the gastric antrum,18 dogs were randomized into the control,HSV and HSV-AMR groups and gastric evacuation,antral myoelectric activity,antral pressure and histological changes were observed 4 omonths after operation.In the mucosa of the reconstruted antrum,no G cells were found.Normal morphology of the stomach and duodenum with distinct gastric peristalese were seen with barium meal study in all the 3 groups.In the HSV-AMR group,the initiation and termination of gastric evacuation were 5.0±0.06 minutes and 4.0±0.4 hours respectively.Which were similar to those of the control and HSV group(P>0.05).The frequency of antral myoelectric action potential was significantyly lower in HSV-AMR group(3.11±0.65 cycles/minute)than in the control and HSV groups(P< 0.05).An intramuscular injection of pentagastrin significantly increaed the antral pressure(P<0.01)in all the groups and eliminated the intergroup difference of pressure(P>0.05).Antral pressure waves occurred immediately after antral myoelectric action potential in a ratio of one to one all the time.The value of antral pressure was the same in the HSV-AMR and control groups(P>0.05)but significantly lower in the HSV group(P<O.05).These findings suggest that the normal motor function is nearly restored in the reconstructed antrum in the HSV-AMR group and in HSV group as well.It is believed that HSV-AMR is the operative procedure of choice for the treatment of chronic duodenal ulcer and for the decrease of its recurrence.
