摘要
目的:探讨载荷下后牙牙尖斜度对上颌全口义齿基托下支持组织压应力分布的影响。方法:建立0°、20°、30°三种牙尖斜度的上颌全口义齿及其支持组织的三维有限元模型,分别进行全牙列、前牙列、单侧及双侧后牙列加载,分析无牙颌各区支持组织所受压应力值大小。结果:全牙列加载时,无牙颌呈现大范围较高压应力区,尤其是前牙区;随牙尖斜度增加,高应力区由前外向后内转移。前牙列加载时,无牙颌前部受压较大,牙尖斜度对压应力值影响不大。双侧或单侧后牙加载,后牙区产生较大压应力,且压应力值与牙尖斜度成正相关关系。结论:加载方式决定了无牙上颌骨表面压应力分布状况,而牙尖斜度对压应力值大小有明显影响,且影响程度与加载方式有关。
Objective: To investigate the influence of cusp inclination of maxillary complete denture on the compressive stress distribution in its supporting tissues. Methods: Three dimensional finite element models were established to simulate maxillary complete denture and its supporting tissues with cusp inclination of 0°、 20°、 30°. The compressive stress distribution was analysed in different areas of edentulous maxilla surface under different loading. Results: The compressive stress was high and comprehensive under complete denture loading, which was higher in the posterior alveolar ridge with the increase of cusp inclination. Cusp inclination didn't have much effect on the stress distribution when anterior teeth was loaded. While applying the load at the bilateral and unilateral posterior molars, stress was higher in posterior alveolar ridge than in the others, and the value was in direct proportion to cusp inclination. Conclusions: Loading conditions affect the compressive stress distribution of the edentulous maxilla surface, and cusp inclinations have a great influence on the stress value.
