小儿先天性心脏病合并肺动脉高压的体外循环管理 被引量：1

摘要 目的总结先天性心脏病合并肺动脉高压(PH)患儿心内直视矫正术中体外循环(CPB)的管理经验。方法对46例先天性心脏病合并肺动脉高压的患者在CPB下行心内直视矫正术,其中轻度PH25例,中度PH13例,重度PH8例。CPB采用浅低温、中度血液稀释、高流量,转流中常规使用超滤技术,及时调整血气指标及电解质和酸碱平衡。结果CPB时间28～180min,全部患儿心脏自动复跳,无死亡和并发症。结论先天性心脏病合并PH的CPB转流中,应特别注意心肌和肺保护,酸碱平衡、电解质紊乱的纠正和血管活性药物应用,以及超滤技术的应用。
