摘要
目的观察野战环境战地两种低张胶体液对腹腔海水浸泡伤实验犬的的救治作用。方法杂种犬24只,随机均分为对照组(A组)、生理盐水救治组(B组)及低张胶体(右旋糖酐70)救治组(C组)及低张胶体(右旋糖酐40)救治组(D组),观察每一组腹腔海水浸泡后的存活时间及血渗透压、乳酸水平变化。结果①C、D两组平均存活时间分别为5.8h、5.9h,显著长于A(2. 4h)、B(4.1h)两组(P<0.01);②腹腔海水浸泡后1hC、D两组血浆渗透压分别为(366.1±8.4)mmol/L、(365.4±7.7)mmol/L,乳酸分别为(8.9±1.8)mmol/L、(8.7±1.7)mmol/L,均显著低于A组[分别为(374.3±11.2)mmol/L、(12.4±3.1)mmol/L](P<0.05)。腹腔海水浸泡后3hC、D组血乳酸水平分别为(9.1±1.7)mmol/L、(9.4±2.3)mmol/L,均显著低于B组(11.4±1.9)mmol/L(P< 0.05)。结论腹腔海水浸泡伤战地早期补入低张胶体液有助于缓解机体高渗状态,改善机体代谢,延长创伤动物生命,两种低张胶体液具有类似作用。
Objective To observe early effect of two kinds of hypotonic colloid solution on celiac seawater immersion wound in field aid. Methods Twenty four dogs with celiac seawater immersion wound were randomly divided into group A (control), saline group B ( treated with saline), group C(treated with dextran 40) and group D(treated with dextran 70). Survival time was observed and plasma levels of osmotic pressure and lactic acid were measured after celiac seawater immersion in every group. Results The survival time of animals was signincantly longer ( P 〈 0.01 ) in groups C (5.8 ± 1.9)h and D (5.9 ± 2.1 )h than that in groups A (2.4±0.6)h and B (4.1 ± 1.2)h. ②The plasma levels pf osmotic pressure (366.1 mmol/L ± 8.4 mmol/L and 365.4 mmol/L ± 7.7mmol/L, respectively) and lactic acid ( 8.9 mmol/L ± 1.8mmol/L and 8.7 mmol/L ± 1.7 mmol/L, respectively) were significantly decreased in group C and D 1 hour after celiac sweater immersion ( P 〈 0.05) compared with group A (374.3 mmol/L ± 11.2 mmol/L and 12.4 mmol/L± 3.1mmol/L,respectively) .The levels of lactic acid in group C and D (9.1 mmol/L ± 1.7 mmol/L and 9.4mmol/L ± 2.3 mmol/L, respectively) were significantly lower than that in group B ( 11.4 mmol/L ± 1.9 mmol/L) 3h after celiac sweater immersion ( P 〈 0.05). Condusion Early infusion of hypotonic colloid solution can alleviate hypertonic conditions, improve metabolism and prolong survival time in the dogs with celiac seawater immersion wound in the field aid. The curative effect of dextran 70 on celiac seawater immersion wound is similar to that of dextran 40 in field aid.
