不同强迫症状的临床特点及与5-HT2A/T102C多态性的关系 被引量:3

Clinical characteristics and association analysis of serotonin 2A receptor gene T102C polymorphism with obsessive compulsive symptoms in three different conditions

摘要 目的探讨强迫症、伴强迫症状的精神分裂症和氯氮平导致强迫症状的精神分裂症三者的症状学差异及与5-HT2A/T102C多态性之间的关系。方法使用Yolle-Brown强迫量表(YBOCS)、汉密顿焦虑量表(HAMA)、汉密顿抑郁量表(HAMD)及阳性和阴性症状量表(PANSS)量表进行临床症状评定;采用聚合酶链式反应-限制性片段长度多态性(PCR-RFLP)技术检测5-HT2A/T102C多态性。结果3组的强迫思维和强迫行为发生率差异具有显著性:强迫症组71.4%和57.1%;精神分裂症伴强迫组66.7%和66.7%;氯氮平导致强迫组33.3%和73.3%;各组5-HT2A/T102C多态性的基因型(TT、TC、CC)和等位基因(T、C)频率间差异无显著性:强迫症组45.7%、40%、14.3%、65.7%和34.3%;精神分裂症伴强迫组46.7%、40%、13.3%、66.7%和33.3%;氯氮平导致强迫组46.7%、40%、13.3%、66.7%和33.3%;正常对照组56.3%、31.3%、12.5%、71.9%和28.1%。结论此三种情况下的强迫症状之间存在症状学差异;5-HT2A/T102C多态性可能不是这三种强迫症状发生的易感因子。 Objective To explore the differences of clinical characteristics and the association of serotonin 2A receptor gene T102C polymorphism with obsessive compulsive symptoms (OCSs) in patients with Obsessive-compulsive disorder(OCD), schizophrenia with OCSs (S + OCSs) and schizophrenia with clozapine-caused OCSs(CLZ + OCSs). Methods All the patients were assessed with Yale-Brown Obsessive-Compulsive Scale (YBOCS) ,Hamilton Anxiety Scale (HAMA), Hamilton Depression Scale (HAMD) and Positive and Negative Symptom Scale (PANSS). The 5-HT2A/ T102C polymorphism was detected with Polymerase Chain Reaction (PCR) and Restriction Fragment Length Polymorphism (RELP) technique. Results The percentages of the obsession and compulsion in the three groups were significant differences: 71.4% and 57.1% in OCD ; 66.7% and 66.7% in S + OCSs; 33.3% and 73.3% in CLZ + OCSs. Genetype ( TT,TC ,CC) and allele ( T、C ) frequencies of 5-HT2A/T102C polymorphism in the three groups and healthy controls were not different significantly: 45.7% , 40%,14.3%,65.7% and 34.3% in OCD; 46.7%,40%,13.3%,66.7% and 33.3% in S+OCSs; 46.7%, 40%,13.3%,66.7% and 33.3% in CLZ + OCSs;56. 3% ,31. 3% ,12. 5% ,71. 9% and 28.1% in healthy controls. Conclusion OCSs in OCD, schizophrenia and that caused by clozapine have symptomatology differences. The 5-HT2A/T102C polymorphism lacks association with OCSs in the three different conditions, which suggests maybe it is not a susceptible gene of OCSs in the three conditions.
