摘要
目的探讨桡骨远端不稳定骨折及关节内骨折快捷、有效、并发症少的手术治疗方法。方法本组48例(56侧)均采用桡骨远端掌侧入路,斜T形纯钛金属板钉(包括LCP)内固定,其中4侧加用了1~2枚克氏针内固定,8侧植入人工骨,10侧注射磷酸三钙骨水泥。术后第一天即进行指问关节活动,第二天开始进行腕关节功能锻炼。结果本组46例(53侧)获得随访,随访时间6-20个月,平均10个月,X线复查结果.骨折全部愈合,关节面平整。参照纽约骨科医院腕关节评价标准综合评价:优33侧,良17侧,可2 侧,差1侧。优良率达94.3%。结论桡骨远端掌侧入路,斜T形纯钛板钉(包括LCP)内固定,骨折端注入磷酸三钙骨水泥是目前治疗桡骨远端不稳定骨折及关节内骨折一种快捷、有效、并发症少的手术方式。
Objective To explore a quick and effective surgical therapy, followed by fewer complications for the instable fracture in distal radius and intra-articular fracture. Methods 48 patients (56 sides) were included in the group, the internal fixation of tilt-T pure titanium metal sheets and screws (including LCP) were performed via volar distal radius of them. In addition, the internal fixation of 1- 2 lag screws was performed at four sides, the artificial bones were implanted at eight sides, and the calcium- tertiary- phosphate bone cement was injected at ten sides. The motion of interphalangeal joints was executed on the 1st day of post - operation; the functional exercise of wrist joints started from the 2nd day. Results The follow- up procedure was carried out in 46 patents of the group, it lasted for 6-20 months, 10 months on average. The complete union of the bone fracture and the even joint facets were found in the results of the re - examination by fluoroscopy in all the patients. According to the Wrist Joint Evaluation Criteria of New York Orthopaedics Hospital, the integral evaluation was performed as follows: excellent at 33 sides, well at 17 sides, acceptable at 2 sides, poor at 1 side; the total rate of good rehabilitation accounting for 94.3%. Conclusion The internal fixation of tilt- T pure titanium metal sheets and screws (including LCP) via volar distal radius, plus the injection of the calcium - tertiary- phosphate bone cement into the facture ends, has currently been proved to be a quick and effective surgical therapy, followed by fewer complications, for the instable fracture in distal radius and intra - articular fracture.
