声襞定位食管入口的解剖学观测

摘要 目的:为置入食管内支架治疗高位食管疾病提供解剖学依据。方法:经福尔马林固定的头颈部尸体标本38例,做正中矢状断面,用1/50mm游标卡尺和精密两脚规测量声襞与环状软骨下缘、会厌游离缘与环状软骨下缘、喉口与环状软骨下缘、环状软骨下缘与第五颈椎(C5)下缘平面之间的距离;并记录环状软骨下缘与颈椎体的相对位置关系。结果:声襞与环状软骨下缘距离(2.391±0.426)cm;会厌游离缘与环状软骨下缘(4.324±0.756)cm;喉口与环状软骨下缘(5.890±0.944)cm;环状软骨下缘与C5下缘距离(-0.761±0.928)cm;环状软骨下缘76.4%位于C6体水平。结论:以声襞定位法放置高位食管内支架,可以充分利用上段正常食管,比颈椎定位法更加准确、合理。 Aim : To supply the anatomy references for intraesophageal stent to treat the upper esophagostenosis. Methods : A total of 38 corpses treated with formalin were broken off along median sagital plane. The distance between the inferior edge of cricoid cartilage and vocal fold, the dissociative edge of epiglottis, the aperture of larynx, and the inferior brim of the fifth cervical vertebrae were determined,respectively. The distance and relative ubiety of the vertebral body and the inferior edge of cricoid cartilage were recorded . Results: The average distance between the inferior edge of cricoid cartilage and vocal fold, the dissociative edge of epiglottis, the aperture of larynx, and the inferior brim of the fifth cervical vertebrae were (2.391±0.426) cm, (4.324±0.756) cm, (5.890±0.944) cm, ( -0.761±0.928) crn, respectively. 76.4% of the inferior edge of cricoid cartilage located at the body of the sixth cervical vertebrae. Conclusion : To mark the upper orifice of esophagus with vocal fold can instruct the placement of upper esophageal stent.
