摘要 目的探讨紫杉醇在青光眼滤过手术中的应用效果。方法将30只新西兰大耳白兔随机分为对照组、实验I组、实验II组3组,双眼均按标准小梁切除术操作,术中巩膜瓣下分别应用0.2mg/ml丝裂霉素C(mitomycin,MMC)、0.3mg/ml紫杉醇5min,实验II组术后第3、6、9d结膜下注射紫杉醇。术前行眼压、ERG检查,术后第4、7、14、28d行ERG检查,术后第4、7、14、28d观察眼压及滤过泡形态。结果实验II组术后第7、14、28d眼压和对照组相比差异有显著性意义(P<0.05),实验I组和对照组相比差异无显著意义(P>0.05);实验II组功能性滤过泡比率较对照组高;各组术后第4、7、14、28d检测的视网膜电流图(ERG)的a波、b波与各组术前进行比较,差异均无显著性意义(P>0.05),30Hz闪烁光反应的振幅在术前和术后各时间段之间进行比较,差异无显著意义(P>0.05)。结论紫杉醇在青光眼滤过手术中应用,术后可降低眼内压,维持功能性滤过泡,紫杉醇在术中及术后联合应用,其作用较MMC强。 Objective To investigate the effect of paclitaxel on the glaucoma filtration surgery. Methods Thirty New Zealand white rabbits were randomly divided into three groups : control group, experimental group Ⅰ, experimental group Ⅱ. Both eyes of rabbits were performed according to the standard of trabaculactomy and placed a piece of cotton with 0.2 mg/ml mitomycin C(MMC) for 5 minutes in control group,0.3 mg/ml paclitaxel for 5 minutes in experimental group Ⅰ, 0.3 mg/ml paclitaxel for 5 minutes in experimental group Ⅱ. In experimental group Ⅱ paclitaxel was by subconjunctival injection at 3,6 and 9 days after surgery. The examination of ERG and intraocular pressure was performed before surgery. The changes of ERG, filtration bleb and intraocular press are were observed at 4,7,14,28 days after surgery. Results The difference of intraocular pressure was significant between experimental group Ⅱ and the control group at 7,14,28 clays after surgery ( P 〈 0.05). As compared with that before treatment, there was no significant difference in ERG at 4,7, 14,28 days after surgery in every group( P 〉 0.05). Conclusion Paclitaxel can reduce effectively intraocular pressure and maintain functional blebs on glaucoma filtration surgery. The efficacy of paclitaxel is more obvious than that of MMC in operation and after operation.
