摘要
目的:研究止血带压力和时间梯度对兔坐骨神经结构的影响。方法:选用14只兔,右后肢为实验组,左后肢为对照组。实验组分为16.0、19.9和24.0kPa(1kPa=7.5mmHg)三个压力梯度,对照组分为26.7、33.3和40.0kPa三个压力梯度。每一压力梯度又分为0.5、1、2和3小时4个时间梯度。结果:组织学发现,压力在26.7kPa以内受压1小时,神经结构不发生明显的改变;超过2小时,有髓鞘和轴突溃变迹象。压力在40.0kPa受压1小时,神经结构出现明显的改变;如达2小时则出现Wallerian溃变。结论:周围神经承受26.7kPa压力2小时,为神经损伤的临界压力。较低压力持续时间延长所造成的神经损伤比高压力短时压迫的损伤要严重的多。
Objective: To investigate the influence of tourniquet of different pressure and time on the structure of sciatic nerve in the rabbits. Methods: The sciatic nerves of 14 rabbits were examined under optical and electron microscope 30 minutes to 14 days after tourniquet compression. The tourniquet pressure was 16.6, 19.9, 24.0kPa (experimental group), and 26.7, 33.3, 40.0 kPa (control group) for varied lengths of time from 30 minutes to 3 hours. Results: The nerve compressed for 1 hour at 24.0 kPa showed very mild structural changes, for 2 hours showed myelin fissure and axonal degeneration, for 1 hour at 40.0 kPa showed obvious structural changes, for 3 hours showed Wallerian degeneration. Conclusions: The nerve compressed for 2 hours at 26.7 kPa is thc threshold of nervc injury. Lower pressure of longer time affects nerve structure more profoundly than higher pressure of shorter time.
