摘要
目的:探讨电视胸腔镜腹腔镜联合下食管癌手术的可行性.方法:术前准备同常规开胸手术.分3步操作,第1步:胸腔内食管游离和淋巴结清扫;第2步:腹腔镜下胃游离及淋巴结清扫;第3步:颈部食管游离及食管胃吻合.结果:本组3例无中转开胸者.手术时间5~7h,术中出血200~400ml,术后均顺利恢复,无明显并发症发生.结论:此手术安全可行,可逐步推广.
Objective: To study the feasibility of using laparoseope and video-assisted thoracoscopic surgery ( VATS ) in radical correction of esophageal carcinoma. Methods:The preparation same as conventional tnoraeotomy. Operation included three stages. First stage: the dissociation of chest section of esophagus and lymph node scavenging under thoraeoscope. Second stage: gastral dissociation and lymph node scavenging under laparoscope. Third stage : the dissociation of fcervical section of esophagus and inosculation of esophagus and stomachus. Results: Three cases was operated under VATS and laparoscope. There was no thoracotomy case . Operating time was in 5-7 hours, average amount of bleeding was in 200-400ml, all cases postoperative recovery were successfull. There was no complicatoin in all cases. Conclusions: This operation is safe and should be applied generally step by step.
