摘要
目的研究腰椎峡部裂及椎体滑脱的手术治疗选择。方法对136例腰椎峡部裂及椎体滑脱患者分别采用改良Scott技术(Ⅰ组,95例)、后外侧融合坚强内固定(Ⅱ组,26例)和后路360°融合内固定(Ⅲ组,15例)治疗。单椎体节段融合73例,二节段融合53例,三节段融合10例。患者平均年龄35.6岁(15～66岁)。结果平均随访3.5年(2～11年),临床疗效示优良率达91.2%,融合成功率为94.9%。结论满意的结果与严格的病例选择,准确、熟练的手术技术和合适的手术方法相关。对腰椎峡部裂及椎体滑脱患者行改良Scott技术、后外侧融合坚强内固定和360°融合内固定均能获得较好的影像学和临床结果。
Objective To study the options for the operation in patients with lumbar spondylolysis and isthmic spondylolythesis. Methods One hundred thirty-six patients with 292 defects were managed with modified Scott＇s technique （group Ⅰ , 95 cases） or autogenous posterolateral art hrodesis （ group Ⅱ, 26 cases） or posteriorly circumferential （360 degree） spinal fusion （group Ⅲ, 15 cases） combined with pedicle screw fixation according to the patient＇s clinical data and the radiographic outcomes. Single - level fusion was performed in 73 patients, two-level in 53 cases and three - level in 10 cases. There were 91 males and 45 females. Their age ranged from 15 to 66 years old with 35.6 years old on average. Results All of 136 cases were followed up for 3.5 years on average. The rate of excellent and good clinical results was 91.2% and the rate of fusion was 94.9%. Conclusion As with any intervention of the three operative methods, a satisfactory outcome often depends on the selection of patients, the performance of precise and skillful surgical technique and the appropriate operative procedure. The successful radiological and clinical outcomes can be achieved in patients with lumbar spondylolysis and isthmic spondylolisthesis with modified Scott＇s technique or the autogenous posterolateral arthrodesis or the posteriorly circumferential （360 degrees） spinal fusion combined with the pedicle screw fixation.
