摘要
目的探讨浅低温心脏不停跳心内直视手术体外循环灌注的临床应用前景。方法46例浅低温心内直视手术,只阻断上、下腔静脉,不阻断升主动脉,不灌注停跳液,建立体外循环后缓慢自然降温至31~33℃,在心脏不停跳的情况下完成心内畸形纠正术。结果全组手术过程和术后恢复均顺利,无心律失常和术后低心排等并发症发生,全部患者均痊愈出院。结论该方法是一种接近生理状态下的体外循环灌注法。
Objective To explore the clinical use of cardiopulmonary bypass with mild hypothermia in open heart operation on beating heart. Methods 46 patients undergoing open heart surgery were obstructed in superior vena eava and inferior vena cava without obstruction in ascending aorta, The operation was done with cardiopulmonary bypass and mild hypothermia (31 - 33℃), The perioperative complication and bypass time were observed and recorded. Results All the patients recovered smoothly without cardiac arrhythmia and low cardiac output syndrome and left hospital healthily. Conclusions Cardiopulmonary bypass without cardiac arrest is similar to the natural state and free from myocardial ischemia and reperfusion injury caused by cardiac arrest, so it can protect heart function maximumly.
