摘要
目的进一步探讨肝纤维化进展与血清标志物水平变化的相关性。方法应用放射免疫、生化和化学发光的方法检测30例正常人和60例肝纤维化患者血清透明质酸(HA)、层粘连蛋白(LN)、IV-C型胶原(IV-C)、Ⅲ型前胶原(PCIII)、肝胆酸(CG)、脯氨酸肽酶(PLD)、单胺氧化酶(MAO)、Ⅰ型前胶原羧端肽原(PICP)八项标志物的水平。结果轻度肝纤维化患者血清HA、IV-C、PCIII、CG的水平明显高于正常人(P<0.01),中度肝纤维化患者血清HA、LN、IV-C、PCⅢ、CG、PICP的水平明显高于正常人(P<0.01),重度肝纤维化患者血清HA、LN、IV-C、PCⅢ、CG、PLD、MAO、PICP的水平都显著高于正常人(P<0.01)。结论血清HA、LN、IV-C、PCⅢ、CG、PLD、MAO、PICP八项指标的联合检测在肝纤维化的诊断中有一定的应用价值,对监测纤维化进展有重要的价值。不同肝纤维化程度与血清这八项标志物均有一定的相关性,但八项血清标志物作为肝纤维化的诊断标准进而取代肝穿病理组织检查的“金标准”有待进一步研究。
Objective To further explore the relationship between the progress of liver fibrosis and the levels of serum laboratory markers. Method Serum levels of hyaluronie acid （HA）, Laminin （LN）, collagen-Ⅳ （IV-C）, collagen- Ⅲ （PC- Ⅲ ）, liver eholie acid （CG）, prolinase （PLD）, monoamine oxidase （MAO）, procollagen-I peptide （PICP） in 30 normal controls and 60 patients with liver fibrosis were measured by radio-immuno-assay and chemiluminescence methods. Results Serum levels of HA, IV-C, PC Ⅲ, CG in patients with mild liver fibrosis were significantly higher than in normal controls （P〈0.01）. Serum levels of HA, LN, IV-C, PCⅢ, CG, PICP in patients with moderate liver fibrosis were significantly higher than those in normal controls （P〈0.01）. Serum levels of HA, LN, IV-C, PC Ⅲ , CG, PLD, MAO and PICP in patients with severe liver fibrosis were significantly higher than those in normal controls （P〈0.01）. Conclusions To combine the detection of serum levels of HA, LN, IV-C, PC Ⅲ, CG, PLD, MAO, PICP has a certain value for liver fibrosis diagnosis, especially for monitoring of liver fibrosis progression. Correlation was also found between the levels of these serum markers and the disease extent of liver fibrosis. Further study is still needed to determine if the serum markers can replace liver biopsy （the ＂golden standard＂） in liver fibrosis diagnosis
