摘要
目的:观察颞颌关节功能锻炼加口香糖法预防鼻咽癌放疗后张口困难的效果。方法:90例经病理确诊为鼻咽癌的患者,随机分为研究组和对照组,研究组45例放疗期间开展颞颌关节功能锻炼加口香糖咀嚼法,对照组45例仅行一般张口练习;以门齿距为观察指标,用SOMA标准评价张口困难程度,组间差异采用t及χ2检验。结果:研究组中张口困难发生11例,Ⅰ级8例,Ⅱ级3例,发生率24·4%;对照组中张口困难发生25例,Ⅰ级17例,Ⅱ级7例,Ⅲ级1例,发生率55·6%;研究组与对照组张口困难发生率比较有显著性差异(P<0·01)。结论:在鼻咽癌患者接受放射治疗的同时,行颞颌关节功能锻炼加口香糖法直至放疗结束,能有效预防放疗所致的张口困难。
Objective： To observe the effect of temporomandibular joint functional exercises and chewing gum to prevent the restriction of jaw opening after radiotherapy of nasopharygeal eareinorna. Methods; 90 patients in pathology proven nasopharygeal carcinoma were randomly divided into two groups： research group and control group. The patients in research group had aeeeped the temporomandibular joint functional exercises and chewing gum, and the patients in control group only had aceeped ordinary exercises of owning mouth. It was evaluated the temporomandibular joint lesion by SOMA： s criterion. The difference between two groups was illustrated with t and x^2. Result： In research group 11 of 45 patients had restriction of jaw opening, 8of 11 had the restriction of Ⅰ degree, 3 of 11 Ⅱ, the rate of restriction was 24.4% ; in control group, 25 of them had restriction of jaw opening, 17of 25 had the restriction of Ⅰ degree, 7 of 25 Ⅱ , 1 of 25 Ⅲ, the rate of it was 55.6 %. There was a statistical significant difference （ P 〈 0.01 ） between the two groups in the rate of restriction opening mouth, Conclusion： It can effectively prevent the restriction of jaw opening due to the radiotherapy that the nasopharygeal carcinoma patients accepted temporomandibular joint functional exercises and chewing gum from the beginning of the radiotherapy to the end.
