摘要
[目的]观察中药善胃方对气阴两虚证型慢性萎缩性胃炎(CAG)胃黏膜异型增生的治疗效果及抑癌基因P16在治疗前后的积分情况。[方法]将内镜和病理确诊为CAG伴异型增生(Dys)和/或肠上皮化生(IM),且符合中医气阴两虚证型的60例患者,随机分为中药治疗组30例和对照组30例。中药治疗组给予善胃方,对照组给予猴头菌片,两组均用药6个月。[结果]中药治疗组治疗前后症状缓解总有效率为90.00%,对照组患者治疗前后症状缓解总有效率为55.67%,P<0.01,病理改变:中药治疗后多数患者病理改善,萎缩、肠化、不典型增生均有改善;而对照组萎缩、肠化、不典型增生改善不明显,中药组明显优于对照组,P<0.05,中药治疗组抑癌基因P16治疗后显著升高(P<0.05)。[结论]善胃方对气阴两虚证型CAG伴Dys和/或IM有良好的治疗作用,并在一定程度上可使异型增生程度逆转,升高抑癌基因P16。
[Objective] To observe the curative effect in patients with chronic atrophic gastritis （CAG） with deficiency of both Qi and Yin and observe the expression of antioncogene protein P16 integral score before and after treatment. [Methods] The selected patients were suffered from chronic atrophic gastritis （CAG） with gastric mucous membrane dysplasia （Dys） and/or intestinal metaplasia （IM） diagnosed by endoscopy and pathologic examination. Sixty patients were randomly divided into treatment group and control group. The patients of treatment group were administrated with Shanwei （invigorating the construction and function of stomach） prescription and the patients of control group with heriacium tablet for six months. [Result] The overall ef- fective rate of treatment group and control group was 90.00% and 55.67% respectively. There was an obvious difference between the treatment group and control group （P〈0.01）. In the treatment group some obvious improvements in the atrophy extent, intestinal metaplasia and atypical hyperplasia were observed（P〈0.05）. The antioncogene protein P16 expression of treatment group was obviously higher than that of control group （P〈0,05）. [Conclusion] The Shanwei prescription has definite curative effect on chronic atrophic gastritis （CAG） with deficiency of both Qi and Yin and may reverse the pathologic change of the gastric mucous membrane on a certain extent by increasln,, the expression level of P16 nrotein nrobablv.
