摘要
【目的】探讨血脂变化与2型糖尿病肾病的关系。【方法】收集本院2001年1月至2004年12月住院的273例2型糖尿病病人,测定其血脂、血糖、尿白蛋白、尿素氮、血肌酐、胰岛素等,根据尿白蛋白排泄率和肾功能将糖尿病患者分为无肾病糖尿病组、早期肾病组、临床肾病组、终末期肾病组,将四组检查结果进行对照比较。【结果】糖尿病并发肾病组(DN组)与无肾病糖尿病组比较:血TG、TC、LP(α)、LDL-C、ox-LDL及HbA1c、FPG、FINS浓度显著升高,而HDL-C浓度显著降低,差异有显著性(P<0.05),其中尤以LP(α)、LDL-C、ox-LDL浓度升高更为突出(P<0.01),且其随糖尿病肾病进展而递增,不同时期糖尿病肾病组间相比差异有显著性(P<0.05)。【结论】脂代谢紊乱是糖尿病肾病的危险因素,尤其是LP(α)、LDL-C、ox-LDL代谢紊乱可能是其独立危险因素。
[Objeetive]To irtvestigate the relation between the change of blood lipids and type 2 diabetic nephropathy. [Methods]There were 273 diabetes mellitus patients in our department from 2001 to 2004. There were 141 cases of diabetes mellitus without nephropathy ,51 cases with early stage diabetic nephropathy, 46 cases with diabetic nephropathy and overt proteinuria,35 cases with end-stage diabetic nephropathy. Serum level of their lipids, blood sugar, urine albumin,serum creatinine, and insulin urea nitrogen were monitored and their levels were compared in the four groups. [Results]The level of serum TG, TC,LP(α),LDL-C ,ox-LDL, HbAlc,FPG,FtNS were significantly increased and HDL-C was decreased in the three groups of diabetic nephropathy,There was significant difference between the group of diabetes mellitus without nephropathy and the three groups with diabetic nephropathy( P 〈0.05). The concentrations of blood LP(α), LDL-C , ox-LDL were significantly higher in the three groups with diabetic nephropathy than in the group of diabetes mellitus without nephropathy,and the levels were obviously increased with the development of the diabetic nephropathy in the three different stages of diabetic nephropathy( P 〈0.05). [Conclusion]Disorder of lipid metabolism is a risk factor of diabetic nephropathy. Disorder of blood LP(α),LDL-C , ox-LDL metabolism may be independent risk factors of diabetic nephropathy.
