摘要
目的应用超声技术观察不同级别高血压患者颈动脉的结构和血流动力学变化及其相互关系。方法将105例原发性高血压按血压水平分为3组,即高血压1级、2级、3级,每组患者各35例与正常对照组35例进行颈动脉超声观察,分别测量各组双侧颈总动脉、颈动脉分叉部、颈内动脉内-中膜厚度(intima-media thick-ness,IMT)、斑块及双侧颈动脉血流动力学指标。结果(1)高血压组的颈动脉内-中膜厚度、斑块发生率明显高于正常对照组(P<0.05),并随高血压分级的增加而增加(P<0.05);(2)高血压组的双侧颈总动脉舒张期末血流速度(end diastolic velocity,EDV)与正常对照组相比有显著性差异(P<0.05)。结论高血压与颈动脉粥样硬化程度有显著相关性,且级别越高,颈动脉硬化程度越重。
Objective The purpose of this study was to observe the structures and blood hemodynamic status of different stages in hypertonsion patients via ultrasound technique. Methods A total of 105 primary hypertension patients were devided into three groups according to hypertension types : the first, second and the third grade groups. Each of the three and the control groups had 35 cases. They all underwent color Doppler ultrasonography. Intima-media thickness (IMT), plaques and hemodynamic indices were respectively measured in both sides of the common carotid artery (CCA), its bifurcation (B) and internal carotid artery (ICA) . Results (1) The incidence of IMT and plaques of the carotid artery was higher in the hypertension groups than that in the control group (P〈0. 05), and increased in the higher grades, (P〈0. 05) . (2) There were significant differences in the end diastolic velocities (EDV). P〈0.05 between the hypertension groups and the control. Conclusions Different hypertension grades were closely correlated with carotid artery atherosclerosis, the higher the grade of hypertension, the severer the carotid artery atherosclerosis.
