摘要
目的分析原发性肺癌患者外周血淋巴细胞及肿瘤细胞染色体异常变化,了解肺癌患者染色体畸变规律。方法应用染色体G显带技术分析50例原发性肺癌住院患者外周血淋巴细胞染色体及55例肺癌肿瘤组织染色体,55例组织标本中鳞状细胞癌24例、腺癌13例、腺鳞癌5例、细支气管肺泡癌8例、小细胞癌4例、非典型类癌1例。结果32%的肺癌患者有外周血淋巴细胞染色体畸变(16/50),显著高于良性病变组和正常对照。中晚期肺癌患者淋巴细胞染色体畸变率为59.1%(13/22),显著高于I、Ⅱ期肺癌患者(10.7%,3/28)。55例新鲜肺癌手术标本常规G显带染色中23例核型分析成功,成功率为41.8%。23例肺癌标本均有明显的染色体异常,除整条染色体丢失外,鳞状细胞癌表现为2q(3/6)缺失,3q(3/6)、3ql3(1/6)和12p(3/6)扩增;肺腺癌为lq(5/6)、5p(2/6)、8q(2/6)扩增,9q(4/6)、10p(2/6)缺失;腺鳞癌为lp(2/3)、6q(l/3)缺失,lq(2/3)、3q(2/3)扩增;细支气管肺泡癌为3p(l/4)、8p(3/4)、11q23缺失,lq(3/4)扩增;小细胞癌为3p(2/3)缺失,3q(2/3)、5p(l/3)、5p11(2/3)扩增。结论原发性肺癌不同病理分型存在广泛的遗传物质不平衡现象,染色体基因扩增和缺失可能是不同类型肺癌发生发展的基础。
Objective To analyse the unbalanced state of heredity material of squamous carcinoma, adenocarcinoma and other type of lung cancer,and to provide base for the studying of the mechanisms of lung cancar. Methods Peripheral lymphocytes of 50 lung cancer patients and 55 specimens of lung cancer from patients suffered from lung cancar were included in this study. G-banding technique was used to analyse the chromosome. Results The aberration rate of chromosome of the lymphocytes in 50 patients were 32 % (16/50), which was significantly higher than those of the benigh disease group and the normal control group. The aberration rateof middle/late stagegroup(59.1%,13/22)was higher than that of the Ⅰ/Ⅱ stage group(10.7%,3/28). Only 23 cases in the total 55 cases had good results in the detecting of the chromosome of lung cancer. In the squamous carcinoma group, the gain region were 3q(3/6),3q13(1/6) and 12p(3/6) ,the loss region were 2q(3/6). In the adenocareinoma group, the gain region were 1q (5/6),5p(2/6),8q(2/6) and 12p(3/6),the loss region were 9q(4/6) and 10p(2/6).Conclusion The heredity material of squamous carcinoma, adenocarcinoma and other type of lung cancer were unbalanced. The extension and deletion of chromatosome was the base of the occurrence of different lung cancer.
