摘要
当前医院改革仍按现行的思路和政策已经走不下去了。从管理学的角度来说,任何事业的正常发展,并获得成功,需要有三个要素,即:坚强的领导、高素质的职工群体和良好的外部环境。环境要素中既有"硬件",也有"软件",它的好坏成了医院能否继续深化改革的关键。我们所讨论的外部环境:一是正确的卫生政策发挥的效应,约束医院的行为,形成有序的运行机制,否则会产生误导;二是健全的医疗保障制度,合理的医疗经费筹措机制,医院的经济补偿稳定,得以正常运转,否则,会迫使医院的行为背离其宗旨,偏离社会保健公平原则;三是整体化的医疗体制,依据卫生模式的战略转变,构建新的连贯性医疗体系,可得到最大的效益;四是高效率的管理。要有高素质的管理人才。这四点都属宏观调控管理,政府和卫生行政部门转变职能,应把目标和工作重点放在这里。
The current hospital reform along the existing idea and policy has come to a dead end. From the angle of management science,the normal progress of any movement with a successful end de- pend on 3 basic elements:a strong leadership,high personnel quality and a good external condition. Such a condition consists of both 'hardware' and 'software'.Whether the condition is favorable or not determines whether it is possible to go on deepening hospital reform.The condition under discussion refers to:1) the positive effect and the restraint on hospital act produced by correct health policies with the formation of orderly operation mechanism;2)a sound health security system with a rational fund— raising mechanism for a timely reimbursement of hospital expenditures to insure normal hospital opera- tion,without which hospitals will be forced to resort to unfair practice of recompensation;3) an inte- gration of medical systems according to the strategic transformation of health model to build a consistent medical program of optimum performance;and 4) a highly efficient management and qualified manage- ment personnel.All these belong to the scope of macro—control on which the government and the health administrative department should focus their effort in the course of their functional transforma- tion.
