摘要
目的评价闭合复位经皮外侧二枚平行克氏针固定治疗儿童不稳定性肱骨髁上骨折的临床效果。方法选取 GarlandⅢ型骨折32例,在“C”型臂 X 光机透视下,闭合整复骨折,经皮外侧二枚平行克氏针固定骨折断端,术后辅以石膏托外固定。结果本组中31例获得随访,平均随访时间为17个月(12~ 22个月),患侧 Baumann 角平均为72.8°,肘关节屈伸功能平均为-7.5°(伸)~135°(屈),无一例发生医源性尺神经损伤、Volkmann 肌挛缩及肘内翻畸形。结论此方法能减少甚至避免医源性尺神经损伤,提供足够结构稳定性防止骨折远端向尺侧再移位,消除肘内翻形成的因素,防止 Volkmann 挛缩的发生,住院时间短,肘关节功能恢复满意。
Objective We respectively reviewed the results of closed reduction and percutaneous two parallel lateral pins of thirty-two of supracondylar fracture in children. Methods All of the fractures were Cartland type Ⅲ in 32 patients and were treated by closed reduction and percutaneous two parallel lateral pins fixation under image intensifier. Plaster was used after operation. Results 31 patients had been follow-up for 12-22 months with an average of 17 months. There were no ular nerve injury, cubitus varus or Volkmann's contraction occurred. X- ray examination revealed an average of Baumann was 72.8 degrees. The mean flexion and extension of the elbow was 7.5°, 135°respectively. Conclusion Closed reduction with percuaneous two parallel lateral pins fixation is perfect for the treatment of fracture. Which is stabilized by the inter fixation that can control reduction and prevent redisplacement of the distal fragments. The elbow can be splinted in a safe position, minimiziing the risk of Volkmann's contraction. The function of the elbow is satisfactory.
