摘要
目的探讨超声实时监视引导下行子宫腔手术操作的临床应用价值。方法对采用超声引导下全程监视子宫腔内手术操作的149例患者的临床及手术资料进行回顾性分析,其中行射频消融凝固治疗术62例(子宫黏膜下肌瘤41例、子宫内膜息肉8例、子宫内膜增殖症13例)、宫内组织残留清宫术67例、异常节育器取出术13例、宫腔粘连伴有宫腔积液行粘连扩张术7例。结果超声引导下149例均一次性完成子宫腔手术,未发生子宫穿孔或其他并发症。结论超声引导下实施子宫腔手术操作使原来复杂、盲目、困难的宫内手术变为简单、安全、准确,可顺利完成的手术。
Objective To determine the clinical value of intrauterine surgery by real-time ultrasound monitoring. Methods Intrauterine operation was successfully performed under the guidance of realtime ultrasound monitoring in 149 patients. Their data were retrospectively analyzed. Among the 149 cases, radiofrequency ablation was performed in 62 cases （ including 41 cases of submucous hysteromyoma, 8 of endometrial polyps and 13 of endometrial hyperplasia） ; uterine curettage and clearing was performed in 67 cases with intrauterine fetal residue; removal of abnormal IUD in 13 cases; and uterine cavity dilatation for adhesion in 7 cases with intrauterine adhesions as well as uterine cavity effusion. Results Intrauterine surgery was performed successfully at one-step under the guidance of ultrasound in all 149 patients. No uterine perforation and other complications occurred. Conclusions By real-time ultrasound monitoring, the original complex, blind and difficult intrauterine procedures can be performed in a simple, safe, accurate and uneventful way.
