母牛分枝杆菌菌苗对结核性胸膜炎患者临床疗效及细胞免疫功能的影响 被引量:2

The clinical effect of M.vaccae on patients with tuberculous pieuritus and its influence on cellular immune function

摘要 目的:观察和评价母牛分枝杆菌菌苗对结核性胸膜炎患者的疗效和对免疫功能的影响。方法:采用随机配对分组法,将64例结核性渗出性胸膜炎分为治疗组(32例)、对照组(32例),均采用2HRZS(E)/4HR化疗方案及抽胸水的方法,治疗组加用母牛分枝杆菌菌苗,对照组加用激素。观察治疗后症状改善、胸水吸收、是否出现胸膜肥厚及包裹性积液、免疫功能改善情况。结果:治疗、对照组患者临床症状改善情况比较,差异无统计学意义(P>0.05);治疗、对照组在2个月、4个月、6个月治疗的显效率(未发生胸膜肥厚及包裹性积液)分别为84.4%与62.5%、93.7%与75%、96.8%与78.1%,治疗组显著优于对照组(P<0.05)。治疗组的细胞免疫功能显著改善,CD3+、CD4+、CD4+/CD8+均升高,与对照组比较有显著差异(P<0.01)。结论:母牛分枝杆菌菌苗在不使用激素的情况下能减少胸膜肥厚、包裹性积液的发生率,调节患者的免疫功能,可用于治疗结核性胸膜炎的辅助治疗。 Objective: To evaluate the effect of Mycobacterium vaecae vaccine on tuberculous pleurisy, .and the change of immunity.Methods:64 cases of tuberculous wet pleurisy were devided to two groups in random,the therapy group(n= 32)and the control group(n= 32). The two groups all were treated by 2HRZS(E)/4HR and excluding pleural effusion, the therapy group received a adjunct treatment of Mycobaeterium vaceae vaccine, the control group received cortisione. Results:There was no statistically significant differences between two groups in clinical symptom.The effective rate after 2 months,4 months,6 months of two groups were 84.4% vs 62.5% ,93.7% vs 75% ,96.8% vs 78.1% respectively. Compared with the control group, the cellular immunity were better in CD3^+ , CD4^+ , CD4^+/CD8^+ in the therapy group. Condusions:Mycobacterium vaccae vaccine could decrease the rate of thicken pleural and encysted effusion. As an adjunct therapy, Mycobactefium vaccae vaccine is effective on tuberculous pleurisy.
