ESAT-6诱导的γ-干扰素反应在HIV合并结核杆菌感染中的价值 被引量:5

摘要 目的探讨ESAT-6体外诱导的γ-干扰素释放反应(简称ESAT-6反应)在人免疫缺陷病毒(H IV)合并结核杆菌(TB)感染中的诊断价值。方法选择H IV(+)TB(+)患者78例,H IV(+)TB(-)患者60例,H IV(-)TB(+)患者30例,健康对照者20例,用全血测定法检测ESAT-6反应。结果ESAT-6反应在H IV(+)患者合并和非合并结核患者中有显著性差异(P<0.01),ESAT-6反应对H IV(+)TB(+)患者检出率明显高于PPD皮试、PPD诱导的γ-干扰素反应。结论我们建立的ESAT-6反应测定法简便、灵敏、特异性高,对H IV合并结核感染患者具有较好的诊断价值。 [Objective] To explore the value of interferon γ(IFN-γ) response induced by ESAT-6 in vitro in mycobacterium tuberculosis (TB) diagnosis among human immunodeficiency virus (HIV) seropositive patients. [Methods] 78 HIV(+)TB(+) patients,60 HIV (+) TB (-) patients, 30 HIV (-) TB (+) patients and 20 normal controls were selected. Heparinized whole blood was incubated at 37℃ for 16-20 hours after ESAT-6 was added. IFN-γ was detected by ELISA method. [Results] The occurrence rate of IFN-γ response induced by ESAT-6 in vitro was much higher than that of PPD skin test or PPD-induced IFN-γ response. There was a significant difference between the HIV (+) TB (+) group and the HIV (+) TB (-) group compared to control groups (P〈0. 01). [Conclusion] This work set up the method of ESAT-6-induced IFN-γ response in vitro. This method is simple, sensitive and specific. It has some value in diagnosis of HIV coinfected TB.
