摘要
目的探讨纤维支气管镜检查术前喷雾法配合环甲膜穿刺法及压缩泵雾化吸入法的麻醉效果。方法对2000年3月至2002年7月应用喷雾法配合环甲膜穿刺法(A组)的312例患者与2002年8月至2005年7月应用压缩泵雾化吸入法(B组)的508例患者的麻醉效果进行比较分析。结果(1)两种麻醉方法的麻醉药耗量相当[(11.7±2.7)m l和(11.3±3.6)m l],麻醉效果优良率分别为96.8%和95.7%(P>0.05)。(2)A组起效时间快[(9.6±1.3)m in],但不良反应率为4.17%,高于B组的0.2%(P<0.01);另外,因穿剌不当,易造成灌洗回收液中混入血液成分,影响细胞分类项目的检查。(3)B组对2、3级气管受压迫的肺癌患者麻醉效果不理想,劣于A组(P<0.05)。结论护士在术前准备时,要做好评估工作,考虑患者综合因素,恰当选择合适的麻醉方法,以求减轻患者痛苦,达到最佳麻醉效果。
Objective To study the effects of two anaesthesia methods before fibrobronchoscopy examination：spraying with cricothyroid membrane puncture and using nebulizer with compressed pump. Methods Group A（ n = 312） used the anaesthesia method of spraying with cricothyroid membrane puncture from March 2000 to July 2002. Group B （ n = 508） used the method of nebulizer with compressed pump from August 2002 to July 2005. The anaesthesia effects of the two groups were compared. Results （ 1 ） The drug doses of the two methods were 11.7±2.7ml and 11.3±3.6ml, with the rate of successful anaesthesia of 96.8% and 95.7% respectively in the two groups （ P〉0.05）. （2） The method used in group A took less time（9.6±1.3 rain）. But the percentage of adverse reaction （4.17%） was higher than the method used in group B （0.2%） （ P〈0.01 ）. There were also some disadvantages in group A. Blood might be mixed into the broncho-alveolar lavage fluid because of inappropriate puncture. This would interfere with the cell classification. （3） The method used in group B did not have an ideal anaesthesia result to lung cancer patients whose tumor were located at the level 2 and level 3 trachea as compared with the method used in group A （ P〈0.05 ）. Conclusion When preparing for the fibrobronchoscopy, nurses should evaluate patients before the operation, take all the factors involved with the operation into account to select the appropriate anaesthesia method in order to alleviate the patient＇s sufferring and to achieve the best anaesthesia results.
