摘要
目的:探讨动脉化门静脉后,肝门部胆管周围血管丛(PBP)内层微血管数量改变对胆管上皮细胞核增殖活性和雌激素受体表达的影响。方法:28只实验大鼠随机分为对照组(A组)、完全动脉化门静脉组(B组)、限流术后完全动脉化门静脉组(C组)、肝门部胆管完全门静脉供血组(D组)。应用免疫组化方法,观察在不同流量与含氧量血液灌流的条件下,PBP内层微血管数以及胆管上皮细胞增生和雌激素受体表达情况。结果:与A组比较,C组、D组PBP内层微血管总数均降低,D组下降最为明显(P<0.01);B组、C组和D组的微动脉数均有下降趋势,其中D组下降最为明显(P<0.01)。与A组相比,各组胆管上皮细胞核增殖指数及雌激素受体表达呈升高趋势,D组上升最为明显(P<0.01)。D组胆管壁组织呈慢性增生性炎症改变。结论:一定流量与含氧量的血液对保证PBP灌注,满足胆管壁组织氧的需求有重要的意义。动脉化门静脉高含氧量与高流量的血液可通过侧支血管逆流灌注PBP,是肝门部胆管壁组织维持正常形态的解剖学基础。
Objective To investigate the microvascular number of hepatohilar peribiliary blood plexus (PBP) after arterialized portal vein (APV) and its influence on cell nuclear proliferation of bile duct epithelium and expression of estrogen receptor (ER). Methods Twenty-eight SD rats were randomly divided into control group (A), APV group (B), APV group after limited operation (C), hepatohilar bile duct group supplied by portal vein blood (D). The inner microvascular number , cell nuclear proliferation of bile duct epithelium and expression of ER were observed with immunohistochemical method under different flow volume and oxygen content blood perfusion. Results Compared with control group, the microvascular number in groups C and D in inner layer of PBP decreased, and it decreased significantly in group D (P〈0. 01) ; the microarterial number in groups B, C and D ininner layer of PBP showed a decreasing trend, and it decreased significantly in group D (P〈0. 01). Compared with control group, the proliferating cell nuclear antigene index (PI) of bile duct epithelium and expression of ER in other groups had a increasing trend, and they increased significantly in group D (P〈0. 01). Tissue of bile duct wall in group D had chronic proliferation inflammation. Conclusion It has important meaning for certain flow volume and oxygen content blood to keep PBP perfusion and satisfy oxygen requirement of bile duct wall tissue. The high flow volume and oxygen content blood in arteried portal vein can full PBP through adverse current of branching vessel, this is anatomical foundation that hepatohilar bile duct tissues keep normal structure.
