摘要
揭示烧伤早期机体细胞分子水平的病理变化,为严重烧伤患者的早期救治提供参考,采用兔30%体表面积烧伤,在伤后即刻,2,4,8,16,24和48小时取血分离中性粒细胞(PMN),用ELISA法测定不同时间内血中PMN粘附蛋白与粘附率的变化及其关系;同时用放免法测定血浆中磷脂酶A2(PLA2)的改变,寻找其与PMN粘附的内在联系。结果:烧伤后PMN粘附蛋白及粘附率均明显高于烧伤前,尤以烧伤后8小时为最显著,然后逐渐下降,至48小时后PMN粘附蛋白和粘附率恢复至烧伤前水平,而兔烧伤后PMN粘附蛋白与粘附率两者之间呈明显的正相关(r=0.90),说明烧伤后PMN粘附蛋白水平的增高是PMN粘附率增强的主要原因之一;烧伤后血浆中PLA2也开始升高,2小时为最明显,推测烧伤后PLA2的变化可能也是PMN粘附性改变的原因之一。
This article explored the pathologic changes at cellular level in the body during the early period of burns and provided the basis for the early treatment of severe burn patients.The blood was collected and the polymorphonuclear leukocytes(PMN)were separated from burn rabbits(30%TBSA) 0hr,2hr,4hr,8hr,16hr,24hr,and 48hr after injury.The PMN adhesion rate and adhesion molecule were measured by the model of PMNbeads and ELISA.Levels of phospholipase A 2 (PLA 2) in plasma were measured by radioimmunoassay.The results showed:the PMN adhesion molecule and adhesion rate after burn were significantly higher than those before burn,particularly at 8hr after the injury,from which time they began to decrease gradually,returning to the preinjury level by 48hr.At the same time,it was found that there was a significant positive correlation between the PMN adhesion molecule and adhesion rate( r =0 90),which indicated that the increase in PMN adhesion molecule was one of the major causes of the increased adhesion rate after burn.Plasma PLA 2 also increased after burn and peaked at 2hr.It is suggested that PLA 2 might also contribute to the changes in PMN adhesion molecule and PMN adhesion rate.
