家兔坐骨神经内注射阿霉素对后肢感觉和运动功能的影响 被引量:6

EFFECTS OF INTRANEURAL INJECTION OF ADRICAMYCIN INTO SCIATIC NERVE TRUNK ON SENSORY AND MOTOR FUNCTION OF HINDLIMBS IN RABBITS

摘要 目的:观察坐骨神经内注射阿霉素对家兔后肢感觉和运动功能的影响,评价其对背根神经节(DRG)的逆行性切除术作用。方法:家兔24只,右侧与坐股韧带相交处的坐骨神经内注射1%阿霉素0.2ml,左侧注射生理盐水作为自身对照。于注射前及注射后1~8周测定双侧下肢痛阈、疼痛回缩反应、下肢运动功能以及坐骨神经运动传导功能。结果:注射阿霉素和生理盐水后1、4和8周双侧坐骨神经运动传导功能与注射前比较无显著性差异(P>0.05)。生理盐水侧1~8周痛阈与注射前比较无显著性差异(P>0.05),疼痛回缩反应正常。阿霉素侧1和2周痛阈低于注射前(P<0.05),4和8周高于注射前(P<0.001);1和2周疼痛回缩反应正常,3~8周疼痛回缩反应逐渐减弱或消失(P<0.01)。1和2周,家兔双侧后肢出现轻微跛行的阳性率无显著性差异(P>0.05),3~8周双侧后肢运动功能正常。结论:坐骨神经内注射小剂量阿霉素能够对相应DRG产生化学切除术效果,坐骨神经支配区域痛觉随阿霉素注射时间的延长逐渐减弱或消失,但下肢运动功能不受影响。 Objective : To observe the sensory and motor function in rabbit hindlimbs after adriamycin intraneural injection into sciatic nerves and evaluate the effects of retrograde adriamycin dorsal root ganglionectomy. Methods: The study involved 24 rabbits. After general anesthesia, 0.2 ml 1% adriamycin was injected into the right sciatic nerves intersecting ischiofemoral ligament, while isodose 0. 9% saline was injected into the same site of the left as control. Pain threshold, withdrawal reflex to pain stimulus,motor function and motor conduction function of bilateral sciatic: nerve were measured before and 1 - 8 weeks after injection. Results : The difference in motor conduction function of bilateral sciatic nerves between before and 1 ,d and 8 weeks after injection was not significant ( P 〉 0. 05 ). Pain threshold of the side injected with saline was maintained and rebound reflex to pain stimulus was normal in 1 - 8 weeks after injection. Pain threshold of the side injected with adriamycin was lower than that of pre-injection in 1 2 weeks (P 〈 0. 05 ) and higher in 3 - 8 weeks ( P 〈 0. 001 ). Moreover, the withdrawal reflex to pain stimulus was normal in 1 - 2 weeks and weakened even disappeared in 3 - 8 weeks (P 〈 0. 01 ). In 1 and 2 weeks, the difference in positive rate of mild lameness in hindlimbs between both sides was not significant (P 〉 0. 05 ). In 3 - 8 weeks, motor function of bilateral hindlimbs was normal. Conclusion : Minidose adriamycin injected into sciatic nerve has the effect of chemo-ganglionectomy on corresponding dorsal root ganglion. The pain perception in the area of sciatic innervation was weakened or disappeared over time while the motor function in hindlimbs was not affected.
