摘要
目的通过观察不同年龄正常皮肤(NS)、增生性瘢痕(HTS)的转化生长因子β1(TGF-β1)表达与胶原构成,探讨正常皮肤胶原构成与年龄的相关性和不同年龄瘢痕易发性。方法采用SABC免疫组化方法观察NS各年龄组:胎儿、1~19岁(儿童青少年组)、20~50岁(成年组)、)60岁(老年组)和HTS组:1~19岁、20~50岁的TGF-β1表达以及Ⅰ、Ⅲ型胶原的含量和比例。结果①HTS组的TGF-β1含量显著大于正常皮肤。1~19岁HTS组TGF-β1表达较20~50岁组的增高具有显著性差异(P〈0.01)。胎儿组皮肤TGF-β1表达低于其余组正常皮肤(P〈0.01);②除1~19岁组与20~50岁组之间的Ⅰ型胶原无差异外,其余各组之间的差异均有显著性意义。)60岁组的Ⅲ型胶原明显低于其他年龄组。正常皮肤Ⅰ/Ⅲ型胶原比例随年龄增加呈上升趋势。HTS组织的Ⅰ/Ⅲ型胶原比例明显高于正常皮肤(NS),在1~19岁HTS组中增高更为显著。结论Ⅰ、Ⅲ型胶原的构成和比例决定正常皮肤的弹性、厚度和张力。TGF-β1表达增高促使增生性瘢痕的形成,1~19岁(儿童青少年时段)HTS组中TGF-β1含量显著增高,导致Ⅰ/Ⅲ型胶原比例失调,可能是该年龄段增生性瘢痕发生率较高的原因之一。
Objective To investigate the relationship of expression of TGF-β1 and collagen in tissues from normal skin and hypertrophic scar in different age periods. Methods The expressions of TGF-β1, type Ⅰ and type Ⅲ collagen of hypertrophic scar in two groups (1 - 19 years old and 20 - 50 years old group) were stained with immunohistochemistry. And the same staining was performed on the normal skin divided into 4 groups-fetal, 1 - 19, 20 - 50 and more than 60 years old. Results The expression of TGF-β1 protein in hypertrophic scar was increased. The expression of TGF-β1 in hypertrophic scar in 1 - 19 years old group was significantly higher than that in 20- 50 years old group (P 〈0. 01 ). The levels of type Ⅰ collagen in all group of normal skin were significant difference except for that between 1 - 19 year old and 20 - 50 year old group. The type Ⅲ collagen of more than 60 years old group was less than the other groups. The ratio of type Ⅰ/type Ⅲ collagen of normal skin was gradually increased with aged. The ratio of type Ⅰ/type Ⅲ collagen of hypertrophic scar was significantly elevated, and that ratio was higher in 1 - 19 years old group. Conclusions Significantly increased TGF-β1 in hypertrophic scar that stimulates more type Ⅰ collagen synthesizing and enhances the ratio of type Ⅰ and type Ⅲ collagen may be one of the causes of higher incidence of scarring in the period of 1 - 19 year old.
