摘要
目的:对心脏瓣膜替换术后死亡患者营养状况进行评价。方法:用回顾性调查法分析147例死亡患者的多项营养指标。结果:体重评价提示约80.0%患者入院时及手术前有营养不良。入院后大部分患者体重降低,入院时平均体重为49.2kg,术前为45.8kg,119例体重下降。多数患者Hb低于正常值,总蛋白和白蛋白在入院之后有所改善。有些患者有电解质紊乱及肝肾功能障碍。单因素Logistic回归模型分析结果提示多器官功能衰竭、肝肾功能不全、营养不良等与心脏瓣膜替换术后患者死亡相关。结论:心脏瓣膜替换术死亡患者在手术前后普遍存在较为严重的营养不良,这可能是影响术后机体恢复、促使术后多种并发症和器官衰竭发生,从而引起死亡的因素之一。
Objective:To evaluate the nutritional status in the dead of 147 cases of inpatients after heart valve replacement.Methods:Multi-items of nutritional indexes in the dead of 147 cases of heart valve replacement were analysed by retrospective study method.Results:The results of single element Logistic regression model suggest that there was a relationship among the malnutrition,multiple organ failure(MOF),liver faillure,renal failure and the heart valve replacement.Conclusion:The data shows that nutritional status is an important factor in the surgical patients with an operation of heart valve replacement,especially malnutrition is a main factor of causing death in inpatient with heart valve replacement in the surgery,and also in MOF.
