人流术后放置LNG-IUS与TCU380A-IUD的临床观察 被引量：11

摘要 目的人流术后妇女多半要求术后采取避孕方法,观察人流术后即刻放置左炔诺孕酮宫内节育器(LNG-IUS)的临床性能。方法采用随机对照临床研究的方法,研究对象为人流产后立即放置IUD的妇女。对120例健康妇女由经过培训的专人于人流术后随机放置LNG-IUS与TCU380A-IUD各60例,于放器后1,3,6和12个月随访。以生命表法统计两种IUD的使用情况。结果因月经量多累计停用率LNG-IUS为0;TCU380A-IUD为6.7%。因闭经累计停用率LNG-IUS为2.1%,TCU380A-IUD为0。因疼痛累计停用率LNG-IUS为1.7%;TCU380A-IUD为5.0%。结论①放置TCU380A-IUD者的出血类型比较恒定,但出血量明显多于LNG-IUS组,后者多为稀发出血、规律的点滴出血或闭经。②LNG-IUS用于流产后避孕耐受性好,续用率高,出血模式更易耐受且术后立即放置减少了宫腔操作次数。因放置前高质量的咨询使女性对LNG-IUS十分了解,故因闭经停用情况非常少。③LNG-IUS与TCU380A-IUD有同样的避孕效果,LNG-IUS要优于TCU380A-IUD,LNG-IUS并且能减少疼痛及盆腔感染。
