摘要
目的：回顾分析旋髂深血管蒂髂骨瓣、髂骨骨膜瓣移植治疗股骨头缺血性坏死和股骨颈骨折不愈合的疗效，探讨手术适应证。方法：治疗股骨头缺血性坏死１４例，股骨颈骨折不愈合５例。切取约６．５ｃｍ×１．５ｃｍ×２．５ｃｍ的髂骨瓣，治疗股骨颈骨折不愈合者另在髂骨内外板处切取２～２．５ｃｍ的骨膜，以便完全用骨膜覆盖股骨颈骨折部位，结果：１１例轻中度股骨头缺血性坏死疗效满意，髋关节功能恢复良好。随访３～５年未见股骨头塌陷，重度组３例症状无明显改善。５例股骨颈骨折不愈合者术后３～５个月骨折愈合，随访２～４年未出现股骨头坏死，髋关节功能恢复满意。结论：旋髂深血管髂骨瓣血运极其丰富，适合于治疗此类疾病，但不宜治疗重度股骨头缺血性坏死。
Objective:To retrospect the results and operative adaptation symptom of avascular necrosis of femoral head and femoral neck nonunion treated by pedicled bone flap with deep iliac circumflex artery,Methods:Total 19 cases,14 cases in avascular necrosis in femoral head and 5 cases in femoral neck nonunions received this operation.According to X-rays, avascular necrosis of the femoral head divided into three groups,(1)slight,5 cases.(2)medium,6 cases(3)severe,3 cases. Results:With the exception of 3 severe cases of avascalar necrosis of femoral head,all the patients obtained satisfactory joint function after 2 to 5 months following-up.The transplanted bones and femoral neck nonunion were healing by first intention, Conclusions:The avascular necrosis of the femoral head(except for severe types)and the femoral neck fracture can be treated by transplanting pedicled bone flap with deep iliac circumflex vessel.
