检测抗CD34抗体标记的肿瘤微血管密度预测肝癌肝移植术后复发 被引量:7

Tumor microvessel density detected by anti-CD34 as a predictor of recurrence of hepatocellular carcinoma after liver transplantation

摘要 目的观察抗CD34抗体标记的肿瘤微血管密度(MVD)对判断肝细胞肝癌(HCC)患者肝移植术后复发的临床意义。方法应用抗CD34抗体进行血管免疫组织化学染色和计算机图像分析系统对82例HCC肝移植患者肿瘤组织中微血管进行标记并计数;结果分析采用logrank检验、Kaplan-Meier分析以及多因素Cox回归分析。结果HCC组织中CD34呈弥漫性、窦隙状阳性表达;肿瘤MVD-CD34与肿瘤大小(P<0.0001)、肿瘤个数(P=0.0101)、有无包膜(P<0.0001)、分期(P<0.0001)、门脉癌栓(P=0.0002)有显著相关性,但与年龄、性别、肝功能分级、淋巴转移、术前血清甲胎蛋白(AFP)含量无显著相关性;肿瘤组织中,高MVD-CD34组与低MVD-CD34组肝移植术后的无瘤生存率比较,差异有统计学意义(P<0.0001),多因素Cox回归分析显示,肿瘤MVD-CD34是一独立的预测HCC复发的预见因子(P=0.0246)。结论肿瘤MVD-CD34与HCC肝移植患者临床病理学特征及术后无瘤生存率密切相关,可独立作为预测HCC肝移植术后复发的重要生物学标记物。 Objective To investigate the clinical significance of tumor microvessel density (MVD) detected by anti-CD34 as a predictor of recurrence of hepatocellular carcinoma (HCC) after liver transplantation. Methods Tissue samples from 82 patients with HCC were immunohistochemically stained with anti CD34 antibody. MVD was evaluated by using CD34. Counting of the microvessels was performed with a computer image analyzer. The survival curves were estimated by KaplanMeier analysis, and the prognostic significance of the marker was analyzed using the log-rank test. Identification of relevant prognostic factors was performed by multivariate Cox regression analysis. Resuits Specific staining of capillary-like vessels by anti CD34 was mainly shown in tumor tissues of HCC. Furthermore, tumor MVD-CD34 was significantly associated with the presence of tumor size (P〈0. 0001 ), number of tumor nodules (P = 0. 0101 ), tumor capsule (P〈0. 0001 ), venous invasion (P = 0. 0002), and tumor stages (P〈0. 0001 ). Patients with high MVD-CD34 had poorer tumor recurrence-free survival than those with low MVD-CD34 (P〈0. 0001 ). Analyzed by a proportional haz ard model, tumor MVD-CD34 was a independent predictor of the tumor recurrence-free survival of HCC after liver transplantation (P = 0. 0246). Conclusions Tumor MVD-CD34 was significantly associated with tumor elinicopathological parameters and tumor recurrence-free survival of HCC. MVD-CD34 may be a feasible marker in predicting HCC recurrence after liver transplantation.
