犬症状性蛛网膜下腔出血脑脊液氧合血红蛋白变化与脑血管痉挛的相关性 被引量:1

Changes of OxyHb in cerebral spinal fluid of symptomatic subarachnoid hemorrhage of canine models and correlation with cerebral vasospasm

摘要 [目的]研究犬症状性蛛网膜下腔出血模型腰大池持续引流脑脊液氧合血红蛋白变化及其与脑血管痉挛的关系。[方法]采用犬蛛网膜下腔出血模型,测定其蛛网膜下腔出血行腰大池持续引流后不同时期,脑脊液氧合血红蛋白浓度,并观察其动态变化,同时应用TCCD检查蛛网膜下腔出血模型不同时期基底动脉流速。[结果]蛛网膜下腔出血后引流组脑脊液中氧合血红蛋白浓度下降明显,腰穿组和对照组脑脊液中氧合血红蛋白浓度轻微下降,引流组和腰穿组及对照组相比,具有显著性差异(P<O.01),同时发现氧合血红蛋白浓度与基底动脉流速呈正相关。[结论]犬蛛网膜下腔出血模型蛛网膜下腔出血后脑血管痉挛与脑脊液中氧合血红蛋白浓度增高有关,腰大池持续引流能有效清除脑血管痉挛因子氧合血红蛋白。 [Objective] To investigate the changes of OxyHb in cerebral spinal fluid (CSF) and the correlation with cerebral vasaspasm in lumbar cistern constant drainage of symptomatic cerebral vasospasm (CVS) following subarachnoid hemorrhage (SAH) in canine models. [Methods] The canine models with symptomatic SAH were set up. The concentrations of OxyHb in CSF were determined in drainage group and non-drainage group and control group of SAH models respectively. TCCD (transcranial color-Doppler) was applied to measure the mean velocity of basal artery blood flow in different time in SAH models. [Results] The concentrations of OxyHb in CSF in drainage group decreased apparently. The content of OxyHb in CSF in non-drainage and control group changed slightly. There were significant difference between drainage group and non-drainage or control group (P 〈 0.01 ). It was found that positive correlation existed between OxyHb in CSF and mean velocity of basal artery blood flow. [Conclusion] The appearance of CVS is correlated to the high concentrations of OxyHb after SAH. CVS caused by oxyHb factor could be removed effectively by lumbar cistern drainage.
