摘要
【目的】评价空心加压螺纹钉治疗老年骨质疏松性股骨颈骨折的疗效及相关注意事项。【方法】回顾分析28例老年骨质疏松性股骨颈骨折患者的临床资料。其中GardenⅠ型5例，GardenⅡ型7例，GardenⅢ型16例，均伴有不同程度的并发病，并发病经治疗稳定后行闭合复位、空心加压螺纹钉经皮内固定术，术后服用中药骨康方煎剂进行骨质疏松病因治疗。【结果】住院期间无死亡病例。经术后平均2．5年的随访，近期并发症3例，远期并发症3例。骨折愈合时间4．5～12．5个月，平均6-8个月。按Harris评分标准，优良率达89．3％。大多数髋关节功能恢复能满足日常生活要求，患者对治疗效果满意。【结论】该法治疗老年骨质疏松性股骨颈骨折具有手术创伤小、操作简便、固定牢靠、骨折愈合率高、可早期功能锻炼、并发症少等优点；其适应症为GardenⅠ型至Ⅲ型的老年股骨颈骨折；由于老年人多有其他并发病，故手术宜待并发病稳定后再进行。
[ Objective] To assess the therapeutic effect of hollow compressing screw for senile osteoporotic femoral neck fractures and to explore the indications. [ Methods] A retrospective analysis was carried out in 28 patients with senile osteoporotic femoral neck fractures. The results of Garden classification of 28 patients with femoral neck fractures were as follows： 5 patients being Garden Ⅰ , 7 Garden Ⅱ and 16 Garden Ⅲ. After the complications of 28 patients were relieved, closed reduction combined with percutaneous hollow compressing screw internal fixation was performed. And etiologic treatment with herbal medicine of Gukang Decoction was given to the patients after operation. [ Results] No dead case was found during the hospitalization. The result of mean 2.5-year follow-up showed that short-tenn complications occurred in 3 cases, long-term complications occurred in 3, the healing time of fracture ranged 4.5 to 12.5 months with an average of 6.8 months, and the excellent rate arrived 89.3% assessed by Harris scoring criteria. The function of most hips was recovered, which ensured the accomplishment of activity of daily life in the patients. [ Conclusion] Hollow compressing screw for senile osteoporotic femoral neck fracture has the advantages of less operative wound, easy operation, firm fixation, higher fracture-healing rate, early functional exercise and less complications. And the indications are： senile osteoporotic femoral neck fracture of Garden Ⅰ and Garden Ⅱ, and the complications being relieved.
