摘要
目的:设计带血供的骨膜瓣转位修复兔关节软骨缺损,以探索小动脉血管蒂骨膜瓣修复关节软骨的可能性。方法:骨刀在髌股关节的股骨侧软骨面制造6mm×8mm软骨缺损,以兔隐动脉关节支为蒂,切取股骨内髁骨膜瓣7mm×9mm,右侧骨膜瓣生发层朝向关节腔覆盖软骨缺损区(实验组),左侧骨膜瓣生发层朝向骨面覆盖软骨缺损区(对照组)。术后2、4、8、12、16周后处死取材,分别作肉眼、光学显微镜及电子显微镜观察。结果:所有骨膜瓣均存活,肉眼观察见两侧新生组织类似关节软骨,光镜和电镜观察见右侧软骨修复优于左侧。结论:带小动脉血管蒂骨膜瓣转位可修复关节软骨缺损,再生软骨接近透明软骨。
Objective: To explore the possibility of vascularized periosteal flap to repair the defect of articular cartilage defect. Methods: Cartilage defect, with the size of 6 mm×8 mm, was made on the articular surface of the femur in the knee joint of rabbits. Periosteal flap of 7 mm×9 mm, formed by periosteum of femoral internal condyle, nourished by articular branch of the saphenous artery was designed. The right periosteal flaps covered the cartilage defect with germinal layer facing joint cavity (experimental group), and the left covered the defect with germinal layer facing the bone surface (control group). After operation of 2, 4, 8, 12 and 16 weeks, rabbits were executed. Tissues were obtained and observed by the naked eye, the optical and the electronic microscope. Results: All flaps survived. Regenerated tissues of both sides looked like articular cartilage. Under optical and electronic microscope, repairing results of left sides seems more better than that of right sides. Conclusions: Periosteal flap pedicled with small artery can be transfered and repaired articular cartilage defect, while the structure of regenerated tissue is similar to transparent cartilage.
