摘要
目的:应用时间-密度曲线分析比较不同的造影剂注射速度对主动脉和肝脏强化程度的影响,以选择适合动脉期和门脉期双期扫描的注射速度和最佳的延迟时间。材料与方法:选取28例正常人随机分为4组,以不同速度(2ml/秒,3ml/秒,4ml/秒及5ml/秒)从周围静脉内注入造影剂后15秒开始扫描,以后每隔15秒在同一层面扫描1次,共3分钟。测量主动脉和肝实质不同时间增强后的CT值,分析时间-密度曲线,并对4组结果做统计学处理。结果:造影剂总量均按1.5ml/kg体重给予时,不同注射速度主动脉和肝实质的强化程度不同,以4ml/秒组最优,但统计学处理和3ml/秒组无差异,而与其他两种速度之间有差异。不同注射速度时主动脉和肝实质到达强化峰值的时间也各不相同,3ml/秒组分别为32.14秒、66.42秒而4ml/秒组分别为30秒、60秒。结论:造影剂总量按1.5ml/kg计算时,延迟时间的选择应根据不同的速度而定。适合双期扫描的速度为3~4ml/秒,而动脉期延迟时间宜用30秒,门脉期为50~60秒。
Objective: Using time density curves analysis for better understanding the effects of alterations in contrast material (CM) injection rate on aorta and liver enhancement, to choose the optimal CM injection rate and delay time in the two phase hepatic helical CT scans.Materials and Methods: Twenty eight patients were randomizely divided into four groups and each group patient received the same amount CM at different injection rate (2ml/s, 3ml/s, 4ml/s and 5ml/s respectively). Scans were performed at the level of the first liver hilus and started 15s following the administration of CM then repeated every 15s for 3min. Aorta and liver enhancement were quantitated on the basis of CT attenuation value of pre and post contrast material. Time density curves were constructed and analyzed statistically.Results: Aortic and hepatic enhancement were different in altering injection rates when the volume of CM was given at 1.5ml/kg. The group of 4ml/s had greater enhancement but no significant difference statistically with the group of 3ml/s, significant difference with the other two groups. It was also found that the mean time reaching to peak aorta and liver enhancement was different at variable rates. It was 32.14s, 66.42s respectively for 3ml/s and 30s, 60s respectively for 4ml/s. Conclusion: The optimal delay time should be varied with CM injection rates when the total volume given was unchanged (1.5ml/kg). 3~4ml/s of injection rate were suitable for dual phase hepatic scans, and its delay time for aterial phase and portal venous scans should be 30 s and 50~60 s respectively. In addition, it should be considered individually depending on the patients condition and scan protocol.
